Abstract

Impaired excretion of sodium and water in patients with cirrhosis, in particular with ascites is a common complication, indicating a poor prognosis associat-ed with increased mortality. Alterations in the renal sympathetic activity, activation of the renin-angiotensin system and secretion of anti-diuretic hormone may account for impaired sodium and wa-ter excretion. Vagal neuropathy, a common com-plication of cirrhosis may lead to similar neuro-humoral abnormalities and contribute to impaired excretion of sodium and water.

Özet

Özellikle asidi olan sirozlu hastalarda sodyum ve su atılımının azalması sık rastlanan ve mortaliteyi ar-tıran kötü bir prognostik faktördür. Renal sempatik aktivitedeki değişimler, renin-anjiotensin sisteminin aktive olması ve antidiüretik hormon sekresyonu, sirozda sodyum ve su atılımında görülen azalmanın fizyopatolojisinin kaynağı olabilir. Sirozda sık kar-şılaşılan bir komplikasyon olan vagal nöropati de aynı nörohumoral anormalliklere sebep olabilir; sod-yum ve su atılımındaki bozulmaya katkıda bulu-nabilir.
