Abstract

DR is a leading cause of acquired blindness among young adults and the working-age population in developed countries. DR is the most frequent microvascular complication of diabet, the prevalence of which increases with the duration of diabetes, with an overall rate of up to 30%. Although severe vision loss may occur due to proliferative DR at diabetic patients, the most common cause of visual loss in these patients is diabetic macular edema(DME). FFA is an invasive, costly, and time-consuming technique but is a sensitive method to detect vascular changes in the course of an es- tablished DR. In contrast to retinography or fl uorescein angiograms, OCT has become the gold standard for the diagnosis, treatment approach, prognosis, assessment of treatment response of patients with DME. OCT angiography(OCTA) is a new noninvasive imaging technique that employs motion contrast imaging to high-resolution volumetric blood fl ow information generating images similar to angiographic images in a matter of seconds . With the widespread use of anti-VEGF, the role of laser photocoagulation in the treatment of DME has decreased, but recent progresses in laser therapy are noteworthy. As a result, anti-VEGF therapy has replaced macular laser therapy and is now the fi rst-line treatment for DME. Intraocular steroids should be considered in pseudophakic, certain groups of patients with systemic vascular comorbidities that are unable to use anti-VEGF or unrespond to anti-VEGF agents.

Özet

Diyabetik Retinopati(DR), gelişmiş ülkelerde genç yetişkinler ve çalışma çağındaki nüfusta edinsel körlüğün önde gelen nedenlerindenbiridir. DR; diyabetin en yaygın mikrovasküler komplikasyonudur, prevalansı diyabet süresi ile birlikte artar ve genel olarak diyabetlile-rin % 30‘unda görülür. Diyabetik hastalarda proliferatif DR(PDR) nedeniyle şiddetli görme kaybı meydana gelebilirse de, bu hastalardagörme kaybının en yaygın nedeni diyabetik makula ödemi(DMÖ)’dir.Fundus Floresein Anjiografi (FFA); invaziv, masrafl ı ve zaman alan bir tekniktir, ancak yerleşmiş bir DR’de vasküler değişikliklerisaptamak için hassas bir yöntemdir. Optik kohorens tomografi (OKT); DMÖ de tanı, tedavi yaklaşımı, prognoz ve tedavi yanıtınındeğerlendirilmesi için altın standart haline gelmiştir. OKT anjiyografi (OKTA), yüksek çözünürlüklü volumetrik kan akımı hareket kont-rast görüntüsünü kullanan, birkaç saniye içinde anjiyografi k görüntülere benzeyen görüntüler üreten yeni noninvaziv bir görüntülemetekniğidir.Anti-VEGF’lerin(vasküler endotelyal büyüme faktörü) yaygın kullanılmaya başlanması ile DMÖ tedavisinde laser fotokoagülasyonunrolü azalmakla birlikte son zamanlarda laser tedavisindeki ilerlemeler de dikkat çekicidir. Sonuçta anti-VEGF tedavi maküler lasertedavisinin yerini almıştır ve artık DMÖ için ilk basamak tedavidir. İntraoküler steroidler psödofakik, anti-VEGF ajanlara yanıt alına-madığı veya anti-VEGF kullanımını kısıtlayan sistemik vasküler komorbiditelerin bulunduğu belli hasta gruplarında düşünülmelidir.
