Analgesia requirement in extracorporeal shock wave lithotripsy treatment and the efficacy of a nonsteroidal antiinflammatory agent

Abstract
Analgesia Requirement in Extracorporeal Shock Wave Lithotripsy Treatment and the Efficacy of a Nonsteroidal Antiinflammatory Agent-The analgesic efficacy of piroxicam, a long acting nonsteroidal antiinflammatory agent, for pain control in patients treated with extracorporeal shock wave lithotripsy (ESWL) for renal stones on an outpatient basis was investigated in a placebo controlled and randomized prospective study. A total of 60 patients were divided into two groups. Forty five minutes prior to ESWL treatment single dose injections of either 2 ml saline (Group Î, n= 20) or 2 ml (40 mg) piroxicam (Group II, n= 40) were given into the gluteal muscle. Verbal rating scale (VRS) was used to evaluate the pain. The two groups were comparable with regard to age, sex, weight, height, stone size, number of shock waves applied and the duration of ESWL treatment (p> 0.05). However, the mean VRS scores were significantly lower in group II than those in group I, both during ESWL treatment and at 6,12 and 24th hours following the ESWL procedure. Opioid analgesics were needed for 4 cases in group I and 1 patient in group II because of unbearable pain (VRS= 5). It seems that analgesia is still needed during ESWL treatment with second generation lithotriptors. Piroxicam has clinically significant effect on this pain and its antiinflammatory effects through inhibition of prostaglandin synthesis provide ureteral relaxation that can help to pass the stones.

— Böbrek taşı nedeniyle ayaktan ekstrakorporeal şok dalga litotripsi (ESWL) tedavisi uygulanacak hastalar-daki ağrının kontrolünde uzun etkili nonsteroidal analjezik antiinflamatuvar bir ilaç olan piroksikamın iyi bir seçenek olup olmadığı plasebo kontrollü, randomize bir prospektif çalışma ile araştırıldı. Toplam 60 hasta iki gruba ayrıldı. Yirmi hastaya 2 mi serum fizyolojik (Grup I), 40 hastaya da 2 mi (40mg) piroksikam (Grup II), ESWL'den 45 dakika önce gluteal kas içine uygulandı. Ağrı değerlendirmesi beşli sözel skala (BSS) yardımıyla yapıldı. Gruplar arasında yaş, boy, cinsiyet, ağırlık, taş boyutu, şok dalgası sayısı ve ESWL süresi bakımından anlamlı fark yoktu (p> 0.05). Piroksikam grubunda, hem ESWL sırasında, hem de altıncı, on ikinci ve yirmi dördüncü saatleri içeren geç dönemde ortalama BSS, plaseboya göre anlamlı olarak düşük bulundu (p< 0.05). Grup l'de 4, Grup ll'de ise 1 hastada dayanılmaz ağrı nedeniyle (BSS= 5) opioid analjezik kullanıldı. İkinci kuşak ESWL cihazlarında da ağrı kontrolünün gerekli olduğu, bu ağrının kontrolünde piroksikamın etkili bir ilaç olduğu kanısına varıldı. Ayrıca, piroksikamın ağrı kontrolü yanında, antiinflamatuvar etkisi ve prostaglandin sentezi inhibisyonu ile oluşturduğu ureter relaksasyonunun taş parçalarının düşürülmesinde ek yarar sağlayabileceği düşünüldü.
