The long-term effects of finasteride and doxazosin in the treatment of benign prostatic hyperplasia

Abstract
A placebo-controlled study was done to investigate the 18-month long term effects offinasteride and doxazosin in the treatment of bladder outlet obstruction and symptoms of benign prostatic hyperplasia. The study included 1 placebo and 2 treatment groups and a total of 100 patients were eligible for final evaluation. There were 46 patients in group 1 who remained on 5 mg/day finasteride for 18 months, 33 patients in group 2 who were given 4 mg/day doxazosin for 18 months and 21 patients in group 3 took placebo for 6 months. The pre- and posttreatment evaluation parameters were symptom scores, maximum urinary flow rates, the amount of residual urine and the prostatic volume. The results obtained in the treatment groups were compared with each other as well as those of the placebo group. The symptom scores and maximum urinary flow rates showed significant improvement in both treatment groups when compared to placebo while the prostatic volume was significantly reduced in the finasteride group only. The therapeutic results obtained in the treatment groups were comparable but a significant difference was present with regard to the prostatic voume decrease. In conclusion, it is evident that both finasteride and doxazosin provided significant improvements in urinary flow rates and symptoms related to benign prostatatic hyperplasia and these effects were sustained in the long term. Even though the efficacy of medical treatment remains far below than that of surgical treatment, it is sufficient for many patients and appears to be superior to the watchful waiting only.

Benign prostat hiperplazisine bağlı semptomların ve mesane çıkımı obstrüksiyonunun tedavisinde finasterid ve doksazosinin 18 aylık uzun dönemde etkilerini incelemek amacıyla plasebo kontrollü bir çalışma yapıldı. İki tedavi ve 1 plasebo grubundan oluşan çalışmada 100 hastanın verileri değerlendirme için uygundu. Birinci grupta 18 ay süreyle 5 mg/gün finasterid verilmiş 46 hasta, ikinci grupta 18 ay süreyle 4 mg/gün doksazo-sin verilmiş 33 hasta ve üçüncü grupta 6 ay süreyle plasebo verilmiş 21 hasta yer almaktaydı. Tüm hastalarda tedavi öncesi ve sonrasında semptom skoru, maksimum idrar akım hızı, rezidüel idrar miktarı ve prostat volümü değerlendirmeleri yapıldı. Finasterid ve doksazosin grubunda tedavi sonrasında elde edilen bulgular plasebo grubuyla ve kendi aralarında karşılaştırıldı. Her iki tedavi grubunda da septom skoru ve maksimum idrar akım hızları plasebo ile karşılaştırıldığında anlamlı düzelme gösterirken, prostat volümü yalnızca finasterid grubunda anlamlı olarak azalmıştı. Finasterid ve doksazosin gruplarında elde edilen düzelmeler karşılaştırıldığında iki grup arasında prostat volümü değişimi yönünden anlamlı fark bulunurken, semptom skoru, maksimum idrar akım hızı ve rezidüel idrar değişimleri açısından fark yoktu. Finasterid ve doksazosin benign prostat hiperplazisine bağlı semptomlarda ve idrar akım hızında anlamlı düzelme sağlamakta ve etkileri uzun dönemde de sürmektedir. Medikal tedavi halen cerrahi tedavi kadar etkili olmaktan çok uzak görünsede birçok hastada yeterli olmaktadır ve izleyerek beklemeye göre çok daha etkili bir seçenek olarak görünmektedir.
