Abstract

The Sigmoidorectal Pouch (Mainz Pouch II)-Several urinary diversion techniques have been described in patients who underwent radical cystoprostatectomy for bladder cancer. Between February 1994 and March 1996, the sigmoidorectal pouch (Mainz pouch II) technique was used in 10 patients following radical cystoprostatectomy. The average follow-up luas 8.7 months (range, 1 to 25 months). Evisceration in 1 and evantration in another patient were the early complications. Late complications toere unilateral ureterocolonic strictures in 2 patients. While ureteral reimplantation was performed in one patient, nephrectomy was done due to chronic pyelonephritic atrophy in the other. Two months after the operation one patient died of multiple metastases and progressive disease. All patients took alkalinizing medication postoperatively and none developed electrolyte imbalance. The mean voiding frequency was 6 times during the day (range, 3 to 7) and twice at night (range, 1 to 4). None of the patients experienced incontinence. The Mainz pouch II appears as an important alternative among continent urinary diversions with its rather simple application that provides a low pressure reservoir with good capacity.

Özet

Mesane tümörü nedeniyle radikal sistoprostatektomi uygulanan hastalarda üriner diversiyon için birçok yöntem tanımlanmıştır. Şubat 1994 ile Mart 1996 arasında kliniğimizde radikal sistoprostatektomi yapılan mesane tümörlü 10 hastaya, üriner diversiyon yöntemi olarak sigmoidorektal poş (Mainz poş II) uygulandı. Hastaların ortalama izlem süresi 8.7 ay (1 ile 25 ay arasında) idi. Erken komplikasyon olarak 1 olguda evisserasyon, 1 olguda evantrasyon görüldü. Geç komplikasyon olarak 2 olguda tek taraflı üreterokolonik anastomoz darlığı gelişti. Bunlardan birine reanastomoz, diğerine ise kronik piyelonefritik atrofi gelişmesi üzerine nefrektomi yapıldı. Bir hasta postoperatif ikinci ayda gelişen yaygın mestastazlar ve progresif hastalık¬la kaydedildi. Tüm hastalar postoperatif alkali solüsyon aldı ve herhangi bir elektrolit dengesizliği ile karşılaşıl¬madı. Hastaların gündüz miksiyon sıklığı ortalama 6 (3 ile 7 arasında), gece ise 2 (1 ile 4 arasında) idi. İnkontinans hiçbir olguda oluşmadı. Düşük basınçlı yeterli rezervuar kapasitesinin bu komplike olmayan kontinen üriner diversiyon yöntemiyle sağlanabilmesi Mainz poş N'nin kontinen diversiyonlar içinde önemli bir alternatif olduğunu göstermektedir.
