What is renal cyst fluid? A biochemical, microbiological and cytopathological evaluation

Abstract
— What Is Renal Cyst Fluid? A Biochemical, Microbiological and Cytopathological Evaluation- Percutaneous aspiration and sderotherapy were done for 27 simple renal cysts in 24 patients. The cystic fluids obtained were examined biochemically, microbiologically and cytopathologically. Biochemical analysis included determination ofMg, Na, Cl, glucose, urea nitrogen, uric acid, creatinin, total cholesterol, total lipid and total protein levels. These parameters were also determined in serum and urine samples of the patients. The cyst fluid cultures remained sterile and cytologic findings were negative with regard to malignancy. The cyst fluid concentrations of urea nitrogen and uric acid were higher than those in the serum but lower than those in the urine. Total lipid levels were found to be lower in the cystic fluid in comparison to the serum levels. The glucose, Mg, Na and creatinine concentrations were comparable in cystic fluid and serum samples of the same patients. In conclusion, renal cyst fluid is neither serum nor urine; it appears as a glomerular filtrate resulting from defective absorption and secretion in the renal tubules.

Toplam 24 hastada saptanan 27 basit böbrek kistine ultrasonografi eşliğinde perkütan aspirasyon ve skleroterapi uygulandı. Elde edilen kist sıvıları biyokimyasal, mikrobiyolojik ve Sitopatolojik olarak değerlendirildi. Biyokimyasal incelemede Mg, Na, Cl, glukoz, üre azotu, ürik asit, kreatinin, total kolesterol, total lipit ve total protein düzeyleri saptandı. Aynı ölçümler hastaların serum ve idrar örneklerinde de yapıldı. Kist sıvısı kültürlerin-de üreme olmadı, sitolojik incelemede malignite lehine bulgu saptanmadı. Kist sıvısındaki üre azotu ve ürik asit konsantrasyonlarının seruma göre daha yüksek, idrara göre daha düşük düzeylerde olduğu saptandı. Kist sıvısı total lipit düzeyleri serum değerlerine göre daha düşük idi. Aynı hastalardaki glukoz, Mg, Na, kreatinin düzeyleri bakımından kist sıvısı ile serum örnekleri arasında fark yoktu. Sonuç olarak, renal kist sıvısı ne serum ne de idrardır, renal tübüllerin bozuk absorpsiyonu ve sekresyonu sonucunda oluşan glomerüler filtrat gibi görünmekte-dir.
