Transplantation and bilateral nephrectomy in giant polycytic kidney disease

Abstract

Objective: To review indications and complications in patients with aittosomal dominant polycystic kidney disease (ADPKD) who underwent renal transplantation and/or bilateral nephrectomy. Methods: In the period between 1995 and 2002, renal transplantations and bilateral nephrectomies were undertaken in five and three patients, respectively, because of ADPKD. Two of the patients underwent ipsilateral nephrectomy at the same session with transplantation due to complaints caused by enlargement of kidney, recurrent infections and technical difficulty in transplantation. Three of the patients underwent bilateral nephrectomy without transplantation because of their symptoms. Results: All of the patients were hypertensive and had a positive family history. Mean age at diagnosis was 29.9 years (14-45) and 42.1 years (23-60) at the time of surgery. The mast common indications for the surgery were pain due to enlarged kidney, recurrent infections and hematuria. The mean operati-ve time was 130 minutes (120-140) for nephrectomy; 181 minutes (175-190) for transplantation; and 250 minutes (240-260) for simultaneous nephrectomy and transplantation. The mean duration of hospitaliiation was 19.7 days (8-45). The mean kid-ney length of the patients who underwent nephrectomy was 33.8 cm (22-48) with a mean weight of 4,245 grams (J,460-7,500). Wound necrosis developed in a diabetic patient and she died because of sepsis in the postoperative second month. Septic pulmonary embolism was treated with intravenous antibiotics in one patient. The complications were observed mainly in the elderly patients with accompanying comorbidities and who have longer been in renal failure necessitating dialysis. Conclusion: Nephrectomy in patients with ADPKD have higher risks than simple nephrectomy not only because of difficulty in surgical technique but also because ofazotemia and infected cysts. Complications are likely to increase with age, presence of comor-bidities and duration of renal failure requiring dialysis. Should they have an appropriate donor, nephrectomy for symptomatic patients can be applied simultaneously with transplantation with minimal increase in surgery time.

Amaç: Otozomal dominant polikistik böbrek hastalığı (ODPBH) nedeniyle bilateral nefrektomi ve/veya transplantasyon yapılan hastaların, nefrektomi indikasyonları ve komplikasyonlannın gözden geçirilmesi. Yöntem: Kliniğimizde 1995 ile 2002 yılları arasında ODPBH nedeniyle beş hastaya renal transplantasyon ve üç hastaya bilateral nefrektomi yapıldı. Transplantasyon yapılan hastalardan ikisine böbrek büyüklüğünden kaynaklanan yakınmaları, tekrarlayan infeksiyonları ve transplantasyonu teknik olarak güçleştirmesi nedeniyle, transplantasyon yapılan tarafa aynı seansta tek taraflı nefrektomi yapıldı. Üç hastaya semptomatik olmaları nedeniyle transplantasyon yapılmaksızın bilateral nefrektomi uygulandı. Bulgular: Hastaların tümü hipertansifti ve tümünde aile öyküsü pozitif bulundu. Ortalama tanı yaşı 29.9 yıl (14-45), cerrahi sırasındaki yaş 42.1 yıl (23-60) olarak bulundu. En sık böbrek büyüklüğünden kaynaklanan ağrı, tekrarlayan infeksiyonlar ve hematüri nedeniyle cerrahi uygulandı. Ortalama ameliyat süresi nefrektomide 130 dakika (120-145); transplantasyonda 181.7 dakika (175-190); nefrektomi ile transplantasyon aynı seansda yapılanlarda 250 dakika (240-260) olarak saptandı. Ortalama yatış süresi 19.8 gün (8-45) idi. Nefrektomi yapılanlarda ortalama böbrek uzunluğu 33.88 cm (22-48) ve ağırlığı 4,245 gram (1,460-7,500) olarak belirlendi. Diyabetik bir hastada yara nekrozu gelişti ve ameliyat sonrası ikinci ayda sepsis nedeniyle kaybedildi. Bir hastada gelişen septik akciğer embolisi intravenöz antibiyotik tedavisiyle düzeldi. Komplikasyonlar yaş, ek hastalık ve diyaliz gerektiren böbrek yetmezliği süresi ile artmaktadır. Sonuç: ODPBH'nda nefrektomi, gerek cerrahi teknikle ilgili güçlükler, gerekse de azotemi, infekte kistler gibi nedenlerle basit nefrektomiden daha riskli bir ameliyattır. Komplikasyonlar yaş, ek hastalık ve diyaliz gerektiren böbrek yetmezliği süresi ile artmaktadır. Uygun organ vericisi bulunan semptomatik hastalarda nefrektomi, transplantasyonla aynı anda cerrahi süresinde minimal uzamayla birlikte yapılabilir.
