The effects of the intensity of endourological training and experience of the urologist on the outcome of ureteroscopic treatment for ureteral stones

Abstract
Objective: To evaluate the effects of the intensity of the endourological training during residency and the surgical experience afterwards on the outcome of ureteroscopic stone treatment. Materials and Methods: The results of the first 30 consecutive procedures performed in a single institution by 4 urologists who had completed different residency programs were reviewed. The success and complication rates were compared among the urologists. Each urologist's results for the first and the latter 15 procedures were also compared. The results for the initial and subsequent 60 cases in the whole group of patients were also compared. Results: There were no significant differences in regard to stone sizes, operation times, success and overall com-plication rates neither between the 30 cases performed by each urologist nor the first and latter 15 cases of each urologist (p> 0.05). However, 75% of the major complications were encountered in the procedures performed by urologist-1. There were significant decreases in the operation time and complication rates between the initial and latter 60 cases (p< 0.05). However, the increase in the success rates for these groups was not significant (p> 0.05). Seventy-one percent of all complications and all of the major complications occured in the initial 50 procedures. Conclusions: There is a relationship between the intensity of the endourological training during residency and occurence of major complications during ureteroscopy afterwards. Fifteen procedures are insufficient for a significant increase in the results. The overall complication rate of the institution significantly improves after 50 ureteroscopies. No such threshold was identified for the success rates.

Amaç: Uzmanlık eğitimi sırasında alınan endoüroloji eğitimininin yoğunluğunun ve sonrasında kazanılan deneyimin Üreteroskopik taş tedavisinde sonuçlara etkisinin araştırılması. Yöntem: Değişik yoğunlukta endoüroloji eğitimi almış 4 cerrahın kliniğimizde uyguladığı ilk 30 üreteroskopi işleminin sonuçlan ve her uzmanın ilk 15 ve ikinci 15 olgudaki sonuçları karşılaştırıldı. Ayrıca tüm grupta ilk ve ikinci 60 olgunun sonuçları kıyaslandı. Bulgular: Dört uzmanın uyguladıkları 30'ar girişim arasında ve ilk ve ikinci 15 girişim arasında taş boyutu, işlem süresi, başarı ve genel komplikasyon oranlan arasından istatistiksel olarak anlamlı fark saptanmadı (p= 0.05). Ancak majör komplikasyonların %75'i ürolog-1 tarafından uygulanan girişimlerde oluştu. İlk ve ikinci 60 girişim için işlem sürelerinde ve komplikasyon oranlarında anlamlı azalma saptandı (p 0.05). Tüm komplikasyonların %71'i ve majör komplikasyonların tamamı ilk 50 girişimde oluştu. Sonuç: Bir ürologun uzmanlık eğitiminde aldığı endoüroloji eğitiminin yoğunluğu ile üreteroskopi sırasında majör komplikasyon gelişmesi arasında bir ilişki saptanmıştır. Onbeş girişim başarı ve komplikasyon oranlarını iyileştirecek deneyimi sağlamada yetersizdir. Kliniğin genci deneyimi olarak 50 girişim sonrasında komplikasyon oranları anlamlı derecede azalmaktadır. Başarı oranları için benzer bir eşik değer saptanamamıştır.
