Radiographic diagnosis and surgical treatment of fragmented coronoid process problem in 17 dogs between 2003-2004

Abstract

Fragmented coronoid process (FCP), is one of the most common cause of forelimb lameness in growing large breed dogs and can be described as the fragmentation of the medial ulnar coronoid process leading to lameness and subsequent degenerative joint disease of the elbow joint. The aim of this study was to evaluate the clinical and radiological diagnosis and the efficiency of surgical treatment of FCP. In this study 17 dogs which was presented to our clinic with intermitant forelimb lameness was evaluated. Surgical treatment of FCP was performed after clinical and radiological examinations. Removal' of the free fragment was performed in cases that have free coronoid process fragmentation and osteotomy of the medial coronoid process was performed in cases that have abnormal cartilaginous structure or fissure formation. Improvement of limb functions was achieved in all cases after the surgery but significant differences between the duration of lameness period were detected, which were associated to the secondary degenerative changes of the elbow joint. As a result, it was determined that physical examination findings should be supported by radiographic studies in the diagnosis of FCP, and proper radiographic positioning had significant importance in detecting secondary degenerative changes in the elbow joint. In this study it is also detected that the success of surgical treatment and duration of postoperative lameness depends on the severity of secondary degenerative changes, proper surgical positioning, non-traumatic surgical procedure and proper postoperative care.

Özet

Fragmented Coronoid Process" (FCP), ulnanm medial coronoid processinin ulnadan ayrılarak topallığa ve dirsek ekleminde dejeneratif eklem hastalığına yol açması olarak tanımlanabilir. Bu çalışmada, özellikle büyük ırk genç köpeklerde en sık gözlenen ön bacak topallığı sebeplerinden biri olan FCP probleminin klinik ve radyolojik tanısı ile bu problemde cerrahi sağaltımın etkinliğinin araştırılması amaçlandı. Çalışma materyalini kliniğimize ön bacak topallığı şikayeti ile getirilen değişik ırk, yaş ve cinsiyette 17 köpek oluşturdu.. Klinik muayeneleri ve radyografik incelemeleri sonrasında FCP problemi saptanan olgulara cerrahi sağaltım uygulandı. Cerrahi sağaltımda, serbest coronoid process fragmenti saptanan olgularda serbest parça uzaklaştırılırken, anormal kıkırdak yapısı ya da fıssur oluşumu saptanan olgularda ise medial coronoid processin osteotomisi gerçekleştirildi. Tüm olgularda cerrahi sağaltımın olumlu sonuç verdiği ancak topallığın ortadan kalkma zamanları arasında belirgin farklılıklar oluştuğu saptandı ve bu durum FCP problemine bağlı olarak dirsek ekleminde oluşan ikincil dejeneratif değişikliklere bağlandı. Bu çalışmada, fragmente medial coronoid process probleminin tanısında fiziksel muayene bulgularının radyografik.incelemelerle desteklenmesinin gerekliliği ve dirsek ekleminde FCP probleminin' ve buna bağlı olarak gelişen .dejeneratif değişikliklerin radyografik olarak görüntülenebilmesi için uygun pözisyonlandırmanın çok önemli olduğu saptanmıştır. Ayrıca, cerrahi sağaltımın başarısının ve topallığın kaybolma süresinin, dirsek ekleminde gelişen ikincil dejeneratif değişikliklerin şiddetine, doğru cerrahi pozisyonlandırmaya, atravmatik cerrahi yaklaşıma ve doğru postoperatif bakıma bağlı olduğu ortaya konmuştur.
