Appendiceal neuroma: A rare cause of acute appendicitis

Abstract

Acute appendicitis is the most common surgical cause of acute abdominal pain. Most commonly seen in 20-30s. The most important factor in the etiology of acute appendicitis is obstruction of the lumen. Lymphoid hyperplasia in children, fecalit in adults and the tumors in elderly were the most common causes of obstruction of the appendiceal lumen. Recently, proliferation of neuroendocrine cells in the stroma of appendix have been implicated in the pathophysiology of acute appendicitis. This situation is called previously with various diagnoses such as fibrous obliteration, obliterated appendix and neurogenic appendix, now the term of appendiceal neuroma is mostly recommended. In this paper, we presented a case of appendiceal neuroma as a rare cause of acute appendicitis.

Özet

Akut apandisit akut karın ağrısının en sık cerrahi gerektiren nedenidir. En sık olarak 20-30’lu yaşlarda görülür. Akut apandisit etiyolojisinde lümen obstrüksiyonu en önemli faktördür. Çocuklarda lenfoid hiperplazi, yetişkinlerde fekalit, yaşlılarda ise tümörler, apandiks lümeninin obstrüksiyonuna en sık sebep olan etkenleri oluşturur. Son zamanlarda apendiks stromasındaki nöroendokrin hücrelerinin proliferasyonunun akut apandisit patofizyolojisinde rol oynadığı saptanmıştır. Bu durum önceleri fibröz obliterasyon, oblitere apendiks, nörojenik apendiks gibi tanılarla adlandırılmakta ise de günümüzde apandisyal nöroma terimi daha fazla kullanılmaktadır. Bu yazıda, akut apandisitin nadir görülen bir nedeni olan apendisyal nöroma olgusunu sunduk.
