Development of inflammatory soft tissue (bursitis) adjacent to an exostosis in a case of diaphyseal aclasis: radiological findings

Abstract
We aimed to present plain radiography, ultrasonography (US), computed tomography (CT), magnetic resonance imaging (MRI) findings of a 48-year-old male with hereditary multiple exostoses, complicated with soft tissue development (bursitis) adjacent to the exostosis in left femur. On radiographs, various findings of diaphyseal aclasis in different bones and a huge exostosis in the distal metaphyseodiaphyseal part of the left femur with adjacent soft tissue swelling were detected. No cortical destruction or pathological soft tissue calcification were observed. On US, surrounding the exostosis, a complicated cystic structure with a thick wall was detected. On CT images, exostosis in the distal part of the left femur and a neighbouring hypodense cystic lesion were seen. On MRI, a cystic lesion with high protein content surrounding the exostosis, which was hyperintense to muscle on T1W images (with minimal peripheral contrast enhancement) and hyperintense on T2W images, was detected. On CT and MRI, no pathological thickening of cartilage cap was observed. Radiological findings were not consistent with sarcomatous malignant transformation. After excision, histopathological diagnosis of the lesion was reported as osteochondroma without malignant degeneration, with an accompanying inflammatory soft tissue mass.

Sol femur distalindeki egzostos komşuluğunda enflamatuvar yumuşak doku (bursit) komplikasyonu olan 48 yaşındaki erkek diyafizyal aklazi olgusunun direkt radyografi, ultrasonografi (US), bilgisayarlı tomografi (BT), ve manyetik rezonans görüntüleme (MRG) bulgularını sunmayı amaçladık. Olgunun radyografilerinde değişik kemiklerde çeşitli diyafizyal aklazi bulguları ve sol femur distal metafizo-diafizer kesim lateralinde büyük boyutlarda bir egzostos ile, komşuluğunda yumuşak doku şişliği tespit edildi. Kortikal yıkım veya bütünlük kaybı, patolojik yumuşak doku kalsifikasyonu saptanmadı. US’de egzostos çevresinde, kalın cidarlı komplike kistik yapı saptandı. BT’de, sol femur distalinde egzostos ve komşuluğunda hipodens kistik lezyon görüldü. MRG’de egzostosu çevreleyen, T1A’da kas dokusuna göre hiperintens ve minimal çevresel kontrastlanma gösteren, T2A’da hiperintens, yüksek protein içerikli kistik lezyon izlendi. BT ve MRG’de kıkırdak kepte patolojik kalınlaşma saptanmadı. Radyolojik bulgular sarkomatöz malign transformasyon ile uyumlu bulunmadı. Eksizyon sonrası lezyonun histopatolojik tanısı, enflamatuvar yumuşak doku kitlesi ile birliktelik gösteren ve malign dejenerasyona uğramamış osteokondrom olarak bildirildi.
