Retrospective Evaluation of Poisonings in Pregnancy in Southeast Anatolia Region of Turkey

Abstract

Objective: The aim of this study is to investigate and analyse pregnant women who were admitted to our intensive care unit due to acute poisoning. Material and Method: All cases were retrospectively collected from our computer records between 10.01.2009 and 10.01.2013; the registration data on age, gestational age, whether the poisoning was deliberate or accidental, causative agent, hospital stay time, education level, Glasgow coma score, use of mechanical ventilation, number of cases per month and mortality outcome were collected and analyzed. Results: Twenty women admitted with acute poisoning followed in intensive care were known to be pregnant and the poisoning was accidental in 30% of the cases, while 70% were suicidal intoxications. Accidental intoxications were due to carbon monoxide and scorpions sting. 64.2% of the suicidal poisonings were caused by medical drugs, with non-steroidal anti-inflammatory, psychiatric drugs and antibiotics being the top culprits and accounted for 44.4%, 33.3% and 22.3% of drug poisonings respectively. 80% of patients were in the 21-34 age groups. In terms of gestational age, 50% of suicidal poisonings were within the second trimester. Mean GCS of patients were 12.8±2.52. Cases were seen particulary in summer and winter. In this study, cases of death have been found. Conclusion: Most cases of acute poisonings during pregnancy were suicidal. Pregnant women attempted suicide mostly within the second trimester of gestation. The most common agents used for suicidal attempt were medical drugs. Our epidemiologic results on this study similar to the literature. Scorpion bites and agricultural materials should be kept in mind in differential diagnosis in our geographic area.

Amaç: Bu çalışmanın amacı zehirlenme nedeniyle yoğun bakıma kabul edilen gebe hastaları araştırmak ve analiz etmektir. Gereç ve Yöntem: 10.01.2009 ve 10.01.2013 tarihleri arasında yoğun bakıma akut zehirlenme ile kabul edilen tüm gebe olgular, bilgisayar kayıtları kullanılarak geriye dönük toplandı; yaş, gebelik haftası, zehirlenme etkeni, zehirlenmenin özkıyım veya kazara oluşu, yatış süresi, eğitim seviyesi, Glaskow Koma Skorları (GKS), mekanik ventilasyon kullanımı, olguların aylara göre dağılımı ve mortalite sonuçları analiz edildi. Bulgular: Yoğun bakımda akut zehirlenme ile takip edilen 20 gebe olgunun %70’i özkıyım amaçlı, %30’u kazara oluşan zehirlenmeler idi. Kazara olan zehirlenmeler karbonmonoksit ve akrep sokmasına bağlıydı. Özkıyım amaçlı zehirlenmelerin %64,2’sinde etken medikal ilaçlar olup sırasıyla %44,4 non steroid anti inflamatuar, %33,3 psikiyatrik ilaçlar ve %22,3 oranında da antibiyotikler saptandı. Olguların %80’i 21-34 yaş grubundaydı. Gebelik haftasına göre, özkıyım amaçlı zehirlenmelerin %50’i ikinci trimesterde olduğu tespit edildi. Hastaların GKS ortalaması 12,8± 2,52 olarak saptandı. Vakalar en çok yaz ve kış dönemlerinde görüldü. Çalışmada bir ölüm vakasına rastlandı. Sonuç: Gebelikteki akut zehirlenme vakalarının çoğu özkıyım amaçlıdır. Gebeler özkıyıma en fazla gebeliğin ikinci trimesterde teşebbüs etmektedir. Özkıyım amacıyla en sık kullanılan madde ise tıbbi ilaçlardır. Bu çalışmadaki epidemiyolojik sonuçlar literatür ile uyumludur. Bölgemizde akrep sokması ile tarım ilaçlarına bağlı zehirlenmelerin ayırıcı tanıda göz önünde bulundurulması gerekmektedir.
