Mitral valve replacement during pregnancy: Case report

Abstract
Thrombosis during pregnancy, due to the change of oral anticoagulant therapy to low molecular weight heparin, is the most common reason for prosthetic heart valve replacement. The aim of the open-heart surgery during pregnancy is to decrease fetal and maternal mortality. Optimal conditions help to minimize fetal mortality during cardiopulmonary bypass. In our case, previous prosthetic mitral valve is replaced because of thrombosis in a 27 week pregnant woman. However, fetus died in the postoperative 24 hour.
Özet
Gebelikde kalp kapakçığı değişimi gerektiren trombüs oluşumunun en önemli nedeni, oral antikoagulan tedavinin düşük moleküler ağırlıklı heparin tedavisi ile yer değiştirilmesidir. Gebelik sırasında açık kalp cerrahisinin amacı, anne ve fetus mortalitesinin azaltılmasıdır. Optimum koşulların sağlanması ile bebek ölümleri en aza indirilmiştir. Bizim olgumuzda, 27 haftalık gebeye prostetik mitral kapakda oluşan trombüs nedeniyle, mitral kapak replasmanı yapılmıştır. Ancak, olguda fetus postoperatif 24. saatte kaybedilmiştir.
