Outcomes of periurethral submucosal injection therapy

Abstract

Objective: to asses the effects of periürethral submucosal injection in stres urinary incontinence due to intrinsic sphincter deficiency. Materials and Method: 23 patients suffering from stres urinary incontinence were included in this study. After the cystometric and pressure-flow studies, ped test, stamey staging, IIQ-7 and UDI- 6 questionnaire were noted before periürethral injection and after one year follow up. Short term postoperative complications and about 1 year of results were compared. Results and Conclusions: Long term side effects of injections weren't reported. In 14 cases, short term haematuria and urethralpain were seen. Ped test showed 44% dryness. When the preoperative and postoperative IIQ-7 and UDI-6 scores were compared, patients showed significant improvement. In selected cases periürethral injection is minimally invazive and efficacious treatment method.

Özet

Amaç: Stres tip idrar kaçırmanın intrensek sfinkter yetmezliğine bağlı olduğu tespit edilen vakalarda, periüretral-submukozal enjeksiyon uygulamasının etkilerini incelemek. Materyal ve Metod: Stres inkontinans semptomu olan 23 hasta çalışmaya dahil edildi. Sistometri ve basınç akım çalışmaları uygulanan hastalara endoskopik periürethral enjeksiyon öncesi ve 1 yıllık gözlem sonrasında, ped test, Stamey evrelemesi, IIQ-7 ve UDI-6 sorgulamaları yapıldı. Kısa dönem postoperatif komplikasyonlar ve ortalama 1 yıllık sonuçlar karşılaştırıldı. Bulgular ve Sonuç: Enjeksiyonların uzun süreli yan etkisi gözlenmedi. 14 olguda kısa süreli hematüri ve ürethral ağrı yakınmaları kaydedildi. Ped testinde %44 oranında tam kuruluk izlendi. Ameliyat öncesi IIQ-7 skorları ile ve UDI-6'nın stres subskalası olan 3.ve 4. soruların skorları, tedaviden 1 yıl sonraki skorlar ile anlamlı derecede iyileşme gösterdiler. Periüretral enjeksiyonlar, iyi seçilmiş hasta gruplarında, minimal invazif hasta memnuniyetini arttıran, etkili bir tedavi metodu olarak gözükmektedir.
