Factors Ajjecting the Survial Rates in Abdominal W all Dejects

Abstract
Background and purpose: Prenatal diagnosis has been reported as benefidal in improving the survival and prognosis after repair of congenital abdominal wall dejects (CA WD 's). However prenatal diagnosis is not the only jactor affecting prognosis in these cases. In this study we aimed to discuss the jactors affecting early (postoperative 1 month) survival rates in cases with cangenifal abdominal wall defects operated at our institution, and relation between survival rates and prenatal diagnosis. Patients and Methods: Clinical records oj the cas es operated with the diagnosis of congenital abdominal wall deject, between January 2004 and May 2012, reviewed retrospectively. Results: During 7 5-year period, 58 cases operated for congenital abdominal wall deject. Of the 58 cases, 29 (50%) had omphalocele, 23 (40%) had gastrochisis and 6 (10%) had umbilical cord hernia. While female!male rates in gastrochisis and omphalocele cas es w ere in favor of females; all umbilical cord hernia cases were male. SurvivaZ rates were decreasing with increasing birth weight in omphalocele cases, and survival rates were correlated positively with birth weight in gastrochisis cases. Survival rat es were higher in prenatally diagnosed gastrochisis cases. There was no dif.ference in survival rates between prenatally diagnosed or undiagnosed omphalocele cases. Survival rat es in omphalocele cases were diagnosed inversely proportional with base diameter of the defect. Highest mortality rates w ere jound if the base diameter was more than 10 cm. There was no associated major cardiac and/or chromosomal anomaly with those omphalocele cases. However, since the sampling po ol was too smail for statistical calculation with student-t and chi square, statistical carreZatian between survivaZ rates and omphalocele base diameter were not significant (P>0,05). Conclusion: With the increasing number of such reported cas es, early diagnosis of associate d cardiac and/or chromosomal anomalies in congenital abdominal wall defect cases, and prediction of the b as e diameter of the defect in omphalocele cases may yield to make predictions and to give more acurate information to the parenis onfuture prognosis and survival rat es of the newborn.

Giriş ve Amaç: Karın ön duvarı defektierinde prenatal tanı konulmasının, prognozu ve sağkalımı olumlu yönde etkilediği bildirilmektedir. Ancak prenatal tanı, karın ön duvarı dejektli olgularda prognozu etkileyen tek faktör değildir. Çalışmamızda, kliniğimizde opere edilmiş karın ön duvarı dejektli olguların postoperalif erken dönemdeki (ilk 1 ay) prognozlarını etkileyen faktörleri saptamak ve bunların prenatal tanı ile iliş kisini tartışmayı amaçladık. Hasta ve Yöntemler: Ocak 2004 ve Mayıs 2012 tarihleri arasında karın ön duvarı defekti tanısı ile opere edilmiş olguların tüm hastane kayıtları geriye yönelik olarak değerlendirildi. Bulgular: 7,5 yıllık sürede kliniğimizde karın ön duvarı defekti sebebiyle o pe re edilen toplam 58 olgudan 29'unda omfalosel (%50), 23'ünde gastroşizis (%40), 6'sında umbilikal kord hernisi (%1 O) mevcuttu. Dişilerkek oranı gastroşizis ve omfaloselde dişi cinsiyet lehine olup umbilikal kord hemisi olguların tümü erkek idi. Omfalosel olgularında doğum ağırlığı arttıkça sağkalım oranlarının azaldığı, gastroşizis olgularında ise kilo ile sağkaZım oranlarının doğru orantılı olduğu saptandı. Prenatal tanılı gastroşizis olgularında sağkaZım oranları yüksekti, omjalosel olgularında tanı alanlar ile almayanlar arasında sağkaZım oranları açısından fark yoktu. Omfalosel grubunda sağkaZım oranlarının, omfalosel taban çapının artması ile ters orantılı olarak de ğiştiği saptandı. En yüksek mortalite, taban çapı 10 cm'nin üstünde olan omfalosel olgularındaydı. Bu olgularda martaliteyi arttıracak ağır kardiak ve/veya kromozomal bir anamali saptanmadı. Omfalosel taban çapı ile erken dönem sağkaZım oranları arasında ters orantılı bir ilişki olduğu; ancak omfalosel grubunda örnek sayısının az olması sebebiyle, bu ilişkinin student-t ve ki kare testi ile değerlendirildiğinde istatistiksel olarak anlamlı olmadığı saptandı (p>0,05). Sonuç; Rapor edilen benzeri olgu sayıları arttıkça, prenatal dönemde tesbit edilen ağır kardiak ve/veya diğer konjenital anamaliterin varlı ğı ve dejekt çapının büyüklüğü, karın ön duvarı dejektli olguların postnatal ve postoperalif erken dönem prognozları ve sağkaZımları üzerine daha doğru öngörüde bulunulmasına ve ebeveynlerinin bilgilendirilmesine yardımcı olabilir.
