Significance of Pretreatment Testosterone Levels in Prostate Cancer Risk Groups

Abstract
Objective: In this report, patients, who had testosterone (TE) level data before prostate needle biopsy and who later underwent radical prostatectomy (RP) due to prostate adenocarcinoma, were evaluated. The TE levels were analyzed in patients divided into three groups according to the D'Amico risk classification.Materials and Methods: Patients in the low-risk group constituted group 1, those in the intermediate-risk group-group 2 and subjects of the highrisk group were in group 3. Prostate specific antigen and TE levels, biopsy and RP pathological findings and biochemical recurrence rates were evaluated. Then all data were compared between the groups.Results: We evaluated 81 patients, whose data on TE level were available. Twenty eight patients were included in Group 1, 45 were in Group 2 and 8 were in Group 3. The mean follow-up time was 37.7 months. Total testosterone (GTE) levels were detected to be lower in Group 3 (p=0.033) than in patients of Group 1 and Group 2. However, pathological stage (p=0.004), Gleason score (p<0.001), tertiary Gleason pattern (p=0.032), tumor volume (p<0.001), surgical margins positivity (p=0.023), lymph node positivity (p=0.01) and biochemical recurrence (p=0.026) rates were found to be higher in Group 3. Conclusion: In conclusion, preoperative low TTE levels in patients who underwent RP were found to be associated with high-risk disease according to the D'Amico risk classification.

Amaç: Bu yazıda kliniğimizde prostat adenokarsinomu nedenli radikal prostatektomi (RP) yapılan hastalar arasından prostat iğne biyopsisi öncesi testosteron (TE) verileri olan hastalar değerlendirildi. D'Amico risk sınıflamasına göre gruplandırılan hastaların TE verileri araştırıldı. Gereç ve Yöntem: Prostat kanseri nedeni ile RP yapılan ve TE verileri olan hastalar D'Amico risk sınıflamasına göre üç grupta değerlendirildi. Düşük risk grubunda olan hastalar Grup 1, orta risk grubunda olan hastalar Grup 2 ve yüksek risk grubunda olan hastalar Grup 3 olarak adlandırıldı. Hastaların prostat spesifik antijen ve TE değerleri, biyopsi ve RP patoloji verileri ve biyokimyasal nüks oranları değerlendirildi. Mevcut veriler Grup 1, Grup 2 ve Grup 3 arasında karşılaştırılarak değerlendirildi. Bulgular: TE verileri olan 81 hasta değerlendirildi. Ortalama 37,7 ay izlemi olan hastalardan 28'i Grup 1'de, 45'i Grup 2'de ve 8'i Grup 3'teydi. Toplam TE (TTE) Grup 3'te düşük saptandı (p=0,033). RP patolojik evresi (p=0,004), Gleason skoru (p<0,001), tersiyer Gleason paterni (p=0,032), tümör hacmi (p<0,001), cerrahi sınır pozitifliği (p=0,023), lenf nodu pozitifliği (p=0,01) ve biyokimyasal nüks (p=0,026) oranları Grup 3'te yüksekti. Sonuç: Sonuç olarak RP öncesi D'Amico risk sınıflamasına göre değerlendirilen hastaların TTE düşüklüğünün yüksek riskli hastalıkla ilişkili olduğu saptanmıştır.
