Craniosinostosis: Treatment and Analysis of 15 Consecutive Cases

Abstract

Objective: Craniosynostosis is a cranial deformity resulting from the premature fusing of one or more sutures in the skull, and is associated with neurological disorders. The goal of the present study is to share an analysis of craniosynostosis surgeries performed at Yuzuncu Yil University Department of Neurosurgery (in Van, Turkey), diagnostic methods, and our experiences of surgical treatments. Materials and Methods: The medical records of 17 infant patients diagnosed with craniosynostosis who underwent surgery between 2013 and 2017 at Yuzuncu Yil University Department of Neurosurgery were examined with respect to age, sex, findings, type of craniosynostosis, type of surgical procedure performed, and complications. Results: Twelve of the patients (75%) were male and 4 were female (25%), with a mean age of 7.6 months (range: 2-10 months). All patients showed evidence of cranial deformity. Thirteen patients exhibited symptoms such as restlessness, insomnia, and head banging. The rate of directed graph diagnosis was 70%, and 100% of the sutures were clearly visible by 3-dimensional computerized tomography. Of the patients, 9 (56.25%) were diagnosed with isolated trigonocephaly, 3 (18.75%) with scaphocephaly, 3 (18.75%) with isolated plagiocephaly, and 1 (6.25%) with plagiocephaly plus trigonocephaly. All patients underwent surgical treatment. Postoperative pneumonia occurred in 1 patient, while 2 patients developed postoperative surgical wound infections. The affected patients were treated for these complications, and there was no incidence of mortality in any of the patients. Conclusion: Craniosynostosis is a rare disease that occurs during the neonatal period. If not treated surgically during the first months of life, the resulting abnormal skull shape may negatively affect the psychological and social development of children. Deformity of the skull and premature closure of the fontanelles should suggest a diagnosis of craniosynostosis. Three-dimensional computerized tomography is the current gold standard for diagnosis, and the primary treatment is surgery.

Özet

Amaç: Kraniosinostoz, kafatasında bir veya birden fazla sutürün normal sürecinden önce kapanması sonucu kafatasında şekil bozukluğu ve nörolojik bozukluklarla seyreden bir hastalıktır. Çalışmanın amacı Van Yüzüncü Yıl Üniversitesi Tıp Fakültesi Nöroşirürji Anabilim Dalın’da yapılan kraniyosinostoz ameliyatlarının analizi, tanı yöntemleri ve cerrahi tedavi deneyimlerimizi paylaşmaktır. Gereç ve Yöntem: Nöroşirürji kliniğinde 2013-2017 tarihleri arasında kraniyosinostoz tanısıyla ameliyat edilen çocuk 16 hastanın dosyaları; yaş, cinsiyet, bulgular, kraniyosinoztoz tipleri, uygulanan ameliyat,komplikasyonlar açısından incelendi. Bulgular: Hastaların 12’i (%75) erkek, 4’ü (%25) kadın, yaş ortalamaları 5,9 (ay) (2-10 ay) idi. Tüm hastalarda kafatası şekil bozukluğu belirgin olarak vardı. 13 hastada çocukta huzursuzluk, uyuyamama, başını etrafa vurma vb belirtiler mevcuttu. Direkt grafide tanı koyma oranı %70, sütürleri net olarak 3 boyutlu gösteren bilgisayarlı tomografide ise %100 olarak bulundu. Hastaların 9’u (%56.25) izole trigonosefali, 3’ü (%18.75) skafosefali, 2’si (%12.5) izole anterior plagiosefali, 1’I (6.25) plagiosefali + trigonosefali, 1'i (6.25) ise posterior plagiosefali tanısı aldı. Hastaların hepsine cerrahi ile tedavi uygulandı. 1 hastada postoperatif pnömoni, 2 hastada ise cerrahi sonrası yara yeri enfeksiyonu, 1 hastada periorbital yağ dokusu herniasyonu gelişti. Bu komplikasyonlar tedavi edildi. Hastaların hiçbirisinde mortalite görülmedi. Sonuç: Kraniyosinostoz yenidoğan çocukluk çağında görülen nadir bir hastalıktır. Yaşamın erken aylarında cerrahi olarak tedavi edilmediği taktirde anormal kafa yapısı ile çocukların psişik ve sosyal gelişimini olumsuz etkiler. Kafatasında şekil bozukluğu ve fontanellerin erken kapanması kraniyosinostoz hastalığını düşündürmelidir. Üç boyutlu Bilgisayarlı tomografi tanıda altın standarttır. Öncelikli tedavi cerrahidir.
