Evaluation of Thyroid Functions in Molar Pregnancies

Abstract

Objective: The aim of the present study was to discuss and compare the clinical outcomes and thyroid functions of complete and incomplete mole pregnancies in the highlights of the literature. Materials and Methods: The study was conducted at a University Hospital, Department of Obstetrics and Gynecology between January 2006 and January 2016. The clinical outcomes and thyroid function tests of 242 complete and incomplete mole pregnancies were reviewed retrospectively. The patients were divided into two groups as, 127 pregnancies with complete mole and 115 pregnancies with incomplete mole. Results: The mean maternal age of complete and incomplete mole pregnancies were 33.87±12.14 and 31.73±10.12, respectively. There was no significant difference between the groups (p: 0.139). The mean TSH values of complete and incomplete mole pregnancies were 0.5±0.9 mIU/ml and 1.0±1.2 mIU/ml, respectively. The mean TSH values were higher in incomplete mole pregnancies than in complete mole pregnancies (p: 0.001). Conclusion: The present study showed that thyroid functions were not affected clinically in mole pregnancies for the follow-up and management of the patients. However, we still recommended that the thyroid functions should be evaluated in mole pregnancies, to prevent the mortal complications of hyperthyroidism.

Amaç: Çalışmanın amacı, kliniğimizde komplet ve parsiyel molar gebelik tanısı konulan olguların klinik verilerini ve tiroid fonksiyonlarını karşılaştırarak, literatür bilgileri ışığında tartışılmasıdır. Gereç ve Yöntem: Bir Üniversite Hastanesinin Kadın Hastalıkları ve Doğum Kliniği' nde Ocak 2006- Ocak 2016 tarihleri arasında, tedavi ve takibi yapılan toplam 242 komplet ve parsiyel molar gebelik olgularının klinik verileri ve tiroid fonksiyon testleri retrospektif olarak incelendi. Hastalar 127 komplet molar gebelik olgusu ve 115 parsiyel molar gebelik olgusu olmak üzere iki gruba ayrıldı. Bulgular: Komplet mol tanısı konulan hastaların yaş ortalaması 33.87±12.14 iken, parsiyel mol tanısı konulan hastaların yaş ortalaması 31.73±10.12 idi. İki grup arasında yaş açısından istatiksel anlamlı fark bulunmadı (p: 0.139). Komplet mol tanısı konulan hastaların TSH değeri ortalamaları 0.5±0.9 mIU/ml iken, parsiyel mol olgularının TSH değeri ortalamaları 1.0±1.2 mIU/ml olarak bulundu. Parsiyel mol olgularının TSH değerleri, komplet mol olgularına göre daha yüksek bulundu (p: 0.001). Sonuç: Çalışmamızın sonuçları değerlendirildiğinde; molar gebelik olgularında klinik takip ve tedavi göz önüne alındığında, tiroid fonksiyon testlerinin anlamlı ölçüde etkilenmediği gözlenmektedir. Bu sonuç, hipertiroidinin ölümcül seyreden komplikasyonları olması sebebiyle, molar gebelik olgularında tiroid fonksiyonlarının değerlendirilmesine gerek olmadığı şeklinde yorumlanmamalıdır.
