Seroprevalence of Hepatitis A and Hepatitis E in Adults Patient Admitted İzmir Katip Çelebi Universty Atatürk Training and Research Hospital

Abstract
Objective: Hepatitis A is the most common form of acute viral hepatitis in the world. Hepatitis E virus (HEV) clinical presentations range from asymptomatic infection to fulminant hepatitis which is frequently seen in pregnant women. The aim of this study is to determine the prevalence of hepatitis A and E transmitted fecal oral in adult population admitted to the hospital. Materials and Methods: It were retrospectively evaluated to test results of the patients admitted to the hospital for various reasons and requested to anti-HAV IgG and anti-HAV IgM, anti-HEV IgG and anti-HEV IgM assay between January December. It was studied by Advia Centaur XP Bayer-Siemens (Germany). Results: It was found as %85.2 (3310/3887) to Anti-HAV IgG seropositivity, %0.51 (20/3887) to Anti-HAV IgM seropositivity, %6.7 (18/270) Anti-HEV IgG seropositivity, %0.7 (2/270) Anti-HEV IgM seropositivity. HAV seropositive patients was admitted 42% to infectious diseases, 22.4% to internal medicine, 25.8 % to other internal branchs, 3.4% to general surgery, 4.5% other surgical branches and 1,9% to emergency service, respectively. HEV seropositive patients was admitted 39% to infectious diseases, 33% to general surgery, 17% to gastroenterology clinics, respectively. Conclusion: In this study was found to Anti-HAV IgG seropositivity was lower than the average of our country and Anti-HEV IgG seropositivity average country in adults. Protection from infection which transmitted fecal-oral rout, especially the beginning of HEV infections; cause of high mortality in pregnant, training, sanitation precautions and environmental hygiene must be actually considered.

Amaç: Hepatit A dünyada akut viral hepatitin en sık görülen şeklidir. Hepatit E virusu (HEV), asemptomatik seyirden gebe kadınlarda sıklıkla görülen fulminant hepatite kadar değişen klinik tablolara neden olabilmektedir. Bu çalışmada, hastanemize başvuran erişkin popülasyonunda fekal oral yolla bulaşan Hepatit A ve E sıklığının araştırılması amaçlanmıştır. Gereç ve Yöntemler: Ocak-Aralık 2012 arasında hastanemize başvuran ve çeşitli nedenlerle Anti-HAV IgG ve Anti-HAV IgM, Anti-HEV IgG ve Anti-HEV IgM tetkiki istenen hastaların test sonuçları retrospektif incelenmiştir. Testler Advia Centaur XP Bayer-Siemens (Germany) ile çalışılmıştır. Bulgular: Anti-HAV IgG seropozitifliği %85,2 (3310/3887), Anti-HAV IgM seropozitifliği %0,51 (20/3887); Anti-HEV IgG seropozitifliği %6,7 (18/270), Anti-HEV IgM seropozitifliği %0,7 (2/270) olarak saptanmıştır. HAV seropozitif hastalar sıklık sırasıyla %42 enfeksiyon hastalıkları, %22,4 iç hastalıkları, %25,8 diğer dahili branşlar, %3,4 genel cerrahi, %4,5 diğer cerrahi branşlar ve %1,9 acil servise başvuran hastalardı. HEV seropozitif hastalar ise %39 enfeksiyon hastalıkları, %33 genel cerrahi ve %17 gastroenteroloji kliniklerine başvuran hastalardı. Sonuç: Çalışmamızda, erişkinlerdeki Anti-HAV IgG seropozitifliği ülkemiz ortalamasından düşük, Anti-HEV IgG seropozitifliği ülkemiz ortalamasında bulunmuştur. Hamilelikte mortalitesinin yüksekliği nedeniyle önemli olan HEV başta olmak üzere fekal-oral yolla bulaflan her iki infeksiyondan korunmada, eğitimle birlikte sanitasyon tedbirleri ve çevre hijyeninin etkin bir biçimde uygulanmasının önemli olduğu düşünülmektedir.
