The Evaluation of the Seroprevalence of HBV, HCV and HIV Infections and the Assessment of Antibody Response to Hepatitis B Vaccination in Hemodialysis Patients

Abstract
Objective: In this study; we aimed to determine the the effectiveness of infection control measures in patients undergoing hemodialysis for chronic renal failure by analyzing the HBsAg, anti-HBs, anti-HCV and anti-HIV serologic indicators retrospectively. Materils and Methods: HBsAg, anti-HBs, anti-HCV and anti-HIV parameters of 96 patients receiving hemodialysis treatment in 2012 were analysed with the ADVIA Centaur XP Immunoassay System by CLIA method. Results: In this study; there was no HBsAg and anti-HIV positive patients. All the patients were regularly and fully vaccinated for hepatitis B but 10 patients (10.4%) had anti-HBs antibody level under 10 mIU/mL. 4 patients (4.1%) were positive for anti-HCV. Statistically significant difference was detected in anti-HCV positivity for hemodialysis duration, gender, history of dental treatment and the presence of hepatitis in the family. Conclusion: In conclusion; appropriate infection control measures in hemodialysis patients, immunization and education of patients and health care workers can reduce the possibility of transmission of infectious agents.

Amaç: Bu çalışmada, kronik böbrek yetersizliği nedeniyle hemodiyaliz uygulanan hastalarda HBsAg, anti-HBs, anti-HCV ve anti-HIV serolojik göstergelerini geriye dönük olarak inceleyerek hemodiyaliz hastalarına uygulanan enfeksiyon kontrol ön- lemlerinin etkinliğinin belirlenmesine yardımcı olunması amaçlanmıştır. Gereç ve Yöntemler: 2012 yılı içinde hemodiyaliz tedavisi gören toplam 96 hastanın HBsAg, anti-HBs, anti-HCV ve anti-HIV parametreleri CLIA yöntemi ile Ad- via Centaur XP Immunoassay System cihazında test edilmiştir. Bulgular: Hastalar içinde HBsAg ve anti-HIV pozitif olan hasta yoktu. Hastaların tümünün hepatit B aşıları tam ve düzenli olarak yapılmasına karşın 10 hastada (%10,4) anti-HBs antikor düzeyi 10 mIU/mLnin altında idi. Toplam 96 hastanın 4ünde (%4,1) anti-HCV pozitifti. Anti-HCV pozitifliğinde hemodiyaliz süresi, cinsiyet, diş tedavi hikayesi ve ailede hepatit varlığı yönünden istatistiksel olarak fark saptandı. Sonuç: Sonuç olarak, hemodiyaliz hastalarında uygun enfeksiyon kontrol önlemleri, bağışıklama, hasta ve sağlık çalışanlarının eğitimi ile enfeksiyon etkenlerinin bulaş olasılığı azaltılabilmektedir.
