The Relationship Between Distribution of HCV-RNA and ALT-AST Levels with Genotypes of Hepatitis C Virus Infected Patients

Abstract
Objective: Approximately 170-300 million people worldwide are estimated to be infected with HCV. HCV is the causative agent about 20% of acute infections and %70 of chronic active hepatitis cases. Asymptomatic course and leading chronicity is the most important problem in HCV cases. The rapid replication of the virus and RNA transcription errors have important role in chronicity. As a result of errors, different genetic sequences of HCV appear. These genotypes 1a, 1b, 2a, 2b, 3a may present worldwide, while the genotypes 4, 5, 6 were only in certain regions. In this study, we investigated HCV genotypes and ALT, AST profiles of newly diagnosed and untreated blood samples obtained from 100 patients positive for HCV RNA admitted to Necip Fazil Kahramanmaras City Hospital microbiology laboratory. Materials and Methods: Blood samples were sent to our laboratory and extracted to yield RNA (Qıagen, Qıasymphony Dsp Virus/Patogen Midi Kit,Germany) and had the RNA quantitation with Rotor-Gene RG-3000 (Corbett Research, Qiagen). Genotyping of samples was carried out with Real Time PCR Reagents Kit (Genome Diagnostics, India). Results: In our multivariable analysis, while evaluating the association of the different genotypes with demographic data, we find a notable difference in genotype distribution among genders and it was obviously seen that genotype 1 was common among older patients, while genotype 3 was common among younger subjects. Conclusion: This study demonstrated the existence of genotype 1 patients highly prevalant (%60) in our study. Although there was no statistically significant relationship between genotype distrubition and HCV RNA load (p>0.005) and AST levels, ALT levels in group of genotype 3 were significantly higher (p=0.0001). Genotype 1 patients were 60% of all and the results of prevalence rates are consistent with other literature in Turkey.
Özet
Amaç: Dünya genelinde yaklaşık 170-300,000,000 kişinin HCV ile enfekte olduğu tahmin edilmektedir. HCV akut enfeksiyonların %20sine ve kronik aktif hepatit olgularının % 70ine neden olmaktadır. Asemptomatik seyir ve kronikleşme; HCV vakalarında en önemli sorundur. Virüsün hızlı replikasyonu ve RNA transkripsiyon hatalarının hızlı çoğalması kronikleşmede önemli bir role sahiptir. Transkripsiyon hatalarının bir sonucu olarak, HCV farklı genetik dizileri görünür. Genotip 4, 5, 6 yalnızca belirli bölgelerde görülebilir iken genotip 1a, 1b, 2a, 2b, 3a ise dünya genelinde gözlenebilmektedir. Bu çalışmada Necip Fazıl Kahramanmaraş Şehir Hastanesi Mikrobiyoloji Laboratuvarına gelen HCV RNA pozitif 100 hastadan elde edilen yeni tanı konmuş ve tedavi edilmemiş hasta kan örneklerinin HCV genotipleri ve ALTAST profilleri incelenmiştir. Gereç ve Yöntemler: Laboratuvarımıza gönderilen kan örneklerinin RNA ekstraksiyonu (Qıagen, Qıasymphony Dsp Virus/Patojen Midi Kit,Almanya) ile ve kantitasyonu ise Rotor-Gene Rg-3000 (Corbett Araştırma, Qiagen) ile yapıldı. Örneklerin genotiplemesi Real Time PCR reaktif kiti (Genome Diagnostics, India) ile gerçekleştirilmiştir. Bulgular: Çok değişkenli analizimizde, demografik veriler ile farklı genotipleri değerlendirirken cinsiyetler arasında genotip dağılımında anlamlı bir birliktelik saptanmış; genotip 3ün erkek hastalar arasında yaygın olduğu görülmüştür (p=0.0001) (Tablo 1). Yine genotip 3 genç hastalarda yaygın görülürken ( Ortalama yaş: 23 ); genotip 1 ise daha yaşlı hastalarda (ortalama yaş: 58,5) gözlendi. Sonuç: Genotip dağılımı ile HCV RNA yükü (p> 0,005) ve AST seviyeleri arasındaistatistiksel olarak anlamlı bir ilişkiye rastlanmamıştır fakat genotip 3 hastalarında ALT seviyeleri anlamlı derecede yüksek görülmüştü (p= 0,0001). Genotip 1 hastalarının; yüksek oranda (% 60) tespit edilmesi de sonuçların ülkemizdeki diğer literatürle uyum göstermekte bu da demografik veri katkısı oluşturmaktadır.
