The influence of patient age, prostate volume, number of biopsy cores and anesthesia time on the pain experienced during trus guided prostate biopsies

Abstract
The possible factors that may affect the degree of pain perceived during transrectal ultrasonography (TRUS)-guided prostate biopsies have been evaluated. Materials and Methods: The pain perceived during TRUS-guided prostate biopsies at the Department of Urology was evaluated according to Visual Analog Scale. Ten ml of 2% Lidocaine HCl, as an anesthetic, was instilled into the rectum either 5 or 10 minutes prior to the insertion of the TRUS probe. No biopsy was performed without anesthesia for ethical reasons. Patients were divided into 4 pain groups according to their pain scores. Groups were compared among each other in terms of patient age, prostate volume, and number of biopsy cores. Furthermore, patients were separated into 2 groups according to anesthesia waiting time, 5 min or 10 min, and they were compared in terms of pain scores. Results: None of the patients described the pain as “intolerable.” Pain groups were found to be similar in terms of age, prostate volume and number of biopsy cores (p>0.05). However, the patients whose anesthesia waiting time was 5 min had significantly greater pain scores (p<0.05). Conclusion: TRUS-guided prostate biopsies with rectal Lidocaine instillation anesthesia were found to be well-tolerated. Waiting for an appropriate time for the anesthesia to take effect, following instillation of Lidocaine would increase patient tolerance. When the anesthesia methods for prostate biopsies are compared, the studies should be standardized in terms of compliance with a proper anesthesia waiting time as well as the anesthetic agent and site for anesthesia.

Transrektal ultrasonografi (TRUS) rehberliğinde prostat biyopsisi biyopsi sırasında duyulan ağrının derecesine etkili olabilecek etkenler araştırıldı. Üroloji Kliniğinde TRUS rehberliğinde prostat biyopsisi yapılan hastaların duydukları ağrı vizüel analog skala ile değerlendirildi. Anestezi için TRUS probunun yerleştirilmesinden 5 ya da 10 dakika önce rektum içine 10 ml %2’lik Lidokain HCl verildi. Etik nedenlerle, anestezi uygulanmayan bir grup oluşturulmadı. Verilen puanlara göre hastalar 4 ağrı grubuna ayrılarak bu gruplar hasta yaşı, prostat hacmi ve biyopsi kor sayısı ölçütleri yönünden birbiriyle kıyaslandı. Ayrıca anestezi bekleme süresine göre hastalar ikiye ayrılarak (5 ya da 10 dk) ağrı skorları yönünden kıyaslandı. Çalışmaya alınan 40 hastadan hiçbiri işlemi dayanılmaz olarak tanımlamadı. Ağrı grupları arasında yaş, biyopsi kor sayısı ve prostat hacmi ölçütleri yönünden istatistiksel olarak anlamlı fark bulunmazken (p>0.05), anestezi bekleme süresi 5 dk olanların ağrı skorları, 10 dk olanlardan anlamlı derecede yüksek bulundu. Rektal Lidokain instilasyonu anestezisi ile yapılan TRUS rehberliğinde prostat biyopsileri tolere edilebilir bulunmuştur. Lidokain instilasyonunun ardından, anestezinin yerleşmesi için uygun bir süre beklenmesi, hasta toleransını artıracaktır. Prostat biyopsileri için kullanılan anestezi yöntemleri kıyaslanırken, kullanılan ajan ve nereye uygulandığının yanısıra, anestezik maddenin etkisinin başlaması için geçmesi gereken süreye uyum yönünden de çalışmaların standardize edilmesi gereklidir.
