Late complications of genitourinary system prostheses

Abstract
There are many reports about early complications of the prostheses performed on genitourinary system. We report the late complications (approximately 7 year) of prostheses observed in our clinic. 100 penile prostheses and 30 artificial urinary sphincters were performed in our clinic between 1985 and 2000. The ratio of early complications of prostheses (in postoperative 3 months) was 6% for penile prostheses and 33% for artificial urinary sphincters. After a mean 7 years follow- up period (5-10 years) the ratio of late complications were 6% for penile prostheses and 16.7% for artificial urinary sphincters. In the patients with penile prosthesis we determined late complications including 2 gonorrhea, 1 migration, 2 fluid leakage and 1 iatrogenic damage. Late complications of artificial sphincters were 2 cuff erosions, 2 fistulas, 1 urethracutaneous abscess, 1 recurrent urinary infection and 1 iatrogenic damage. Some complications may appear in time using these genitourinary prostheses. So we have to follow our patients with prosthesis regularly. All practioners in medicine should be informed about prostheses in genitourinary system.

Genitoüriner sisteme uygulanan protezlerin erken dönem komplikasyonlarından çok bahsedilmiştir. Biz, bu protezlerin uzun vadede (ortalama 7 yıl ) neler ortaya çıkarabileceğini kliniğimize müracaat eden dramatik vakalarla anlatmaya çalıştık. Kliniğimizde 1985-2000 yıllan arasında 100 penil protez ve 30 artifisyel üriner sfinkter uygulaması yapılmıştır. Peroperatuvar ve erken dönem (3 aylık takip sonrası) komplikasyon oranı her tip penil protez için %6, artifisyel üriner sfinkter için %3.3'tür. Ortalama 7 yıllık (5-10 yıl) takip sırasında geç dönem komplikasyon oranı penil protezler için %6 (6 vaka), artifisyel üriner sfinkter için %16.7 (7 vaka)'dir. Uzün süreli takipte penil protezli hastalarda; 2 gonore, 1 migrasyon, 2 sıvı kaybı ve 1 iyatrojenik hasarlanma tespit ettik. Artifisyel üriner sfînkterli hastalarda ise; 2 kaf yırtığı, 2 fistül, l üretrokutenoz abse, 1 tekrarlayan üriner enfeksiyon ve l iyatrojenik hasarlanma tespit ettik Genitoüriner sistem protezleri uzun dönemlerde sorunlar çıkartabilir. Düzenli olarak hasta görülmeli ve protezler tüm hekimlere tanıtılmalıdır.
