Different two vesicourethral anastomosis techniques in laparoscopic radical prostatectomy: Match-pair analysis

Abstract

Introduction: Because of the epidemiologic importance of prostate cancer, laparoscopic radical prostatectomy aroused considerable interest in recent years. One of the major controversies concerning this technique is the long operating time. As in open surgery, the vesicouretral anastomosis is one of the critical steps of the surgical procedure. However, for ergonomic reasons, the techniques used in retropubic or perineal radical prostatectomy are not optimal during laparoscopic surgery because knotting is exceedingly time consuming. Fort this reason, the technique of running sutures using henicircumferential sutures for vesicourethral anastomosis were described, requiring two knottings intracoporeally. Recently, a modified technique including only one intracorporeal knot-tying were emerged which is highly decrease anastomosis time in laparoscopic radical prostatectomy. We analyze herein the running and interrupted suture techniques using match-pair analysis. Materials and Methods: We reviewed the charts of 28 consecutive patients who underwent selectively running sutures for vesicourethral anastomosis (Group 1) comparing it to 28 match-paired patients in whom the anastomosis created by interrupted suturing (Group 2) in laparoscopic radical prostatectomy. The patients were matched for prostate volume (grams), laparoscopic access (transperitoneal or extraperitoneal), presence of pelvic lymph node dissection, type of nerve-sparing and bladder neck sparing technique, and additionally pathologic stage and surgical positive margins. In addition to age, preoperative PSA value (ng/ml), tumor volume (cc.) and Gleason score, perioperative parameters (operating time, anatomosis time, blood donation) and postoperative catheterization time were analyzed. Results: Patients were 64.4±7.8 versus 62.8±6.5 years old (p: 0.906), had 9.9±7.5 versus 7.5±6.1 ng/ml PSA (p: 0.334) in Group 1 and Group 2, respectively. The mean prostate volume, tumor volume and Gleason score were 43.4±13.7 grams, 2.7±1.8 cc. and 6.3±0.7 in the Group 1, 44.2±17.0 grams, 3.2±3.5 cc. and 6.1±1.1 in the Group 2. There was no statistical significance for those parameters. A total of 10 pelvic lympadenectomy and 17 nerve-sparing (4 unilateral and 13 bilateral) procedures were performed in each group. Pathologic stages were T2a in 2, T2b in 4, T2c in 12, T3a in 8, T3b in 1 and T4a in 1 patients for both groups (p:1.000). The mean operating time with 183.1±45.2 vs. 210.2±36.8 minutes and the mean anastomosis time with 21.5±7.1 vs. 27.5±7.0 were significantly longer in Group-1 when compared Group-2 (p:0.008). In addition, there was statistical difference of median catheter time (7 vs. 8.5 days, p: 0.007). Whereas no blood donation was required, there was no complication in each group depend on anastomosis techniques. Conclusion: In the creation of vesicourethral anastomosis which is the critical step of laparoscopic radical prostatectomy, modified circumferential running suture including only one intracorporeal knotting is safe and shorter than that of interrupted suture technique. With running sutures not only operation time but also postoperative urethral catheterization time are significantly become shorter.

Son yıllarda tüm dünyada laparoskopik radikal prostatektomi giderek artan oranlarda uygulanmaya başlanmıştır. Öğrenme süresindeki zor ve uzun olmasının en önemli nedenlerinden biri ablatif bölüm olan prostatektomi kadar rekonstrüktif bölüm olan üretrovezikal anastomozu gerçekleştirmedeki sıkıntılardır. Bu çalışmada son yıllarda popülerize olan kontinü anastomoz tekniği ile tek tek sütür tekniğine göre yapılan anastomozun özellikleri çapraz eşleşmeli analiz ile değerlendirilmesi planlandı. Kontinü sütür tekniği ile anastomoz yapılmış ardışık son 28 olgunun (Grup 1) değerleri, çapraz eşleştirme analizi ile belirlenen ve tek tek sütür tekniği ile anastomoz gerçekleştirilmiş 28 hastanın değerleri ile karşılaştırıldı. Tek tek sütür anastomoz tekniği uygulanan 1018 hasta içinden prostat ağırlığı, uygulanan yaklaşım yolu (transperitoneal ve ekstraperitoneal), aynı seansta pelvik lenfadenektomi, sinir koruyucu yaklaşım ve mesane boynu koruyucu yaklaşım uygulanması yanında, patolojik evre, cerrahi sınır pozitifliği parametrelerine göre oldukça selektif şekilde çapraz eşleştirme analizi ile 28 olgu seçildi (Grup 2). Her iki gruptaki hastaların ameliyat ve anastomoz süresi ile uretral kateterizasyon zamanı değerlendirildi. Her iki gruptaki hastaların yaş, prostat ağırlığı, tümör volümü ve Gleason skorları arasında istatistiksel açıdan fark yoktu (sırasıyla, p: 0.691, p: 0.252 dakika, p: 0.344). Grup 1’de, Grup 2’ye oranla, ortalama ameliyat süresi (183.1±45.2 vs. 210.2±36.8 dakika, p: 0.008) ve anastomoz süresi (21.5±7.1 vs. 27.5±7.0; p: 0.008) anlamlı olarak daha kısa olarak belirlendi. Bunun yanında Grup 1’deki hastalarda ortalama ve medyan üretral kateterizasyon kalış süresi de anlamlı şekilde daha kısa olarak tespit edildi (7.0 vs. 8.5 gün; p: 0.007). Her iki grupta da anastomoz tekniğinden ileri gelen komplikasyon oluşmadığı gibi hiçbir hastaya ameliyat sırasında yada sonrasında kan transfüzyonu yapılmadı. Laparoskopik radikal prostatektominin önemli bir parçasını oluşturan üretrovezikal anastomozun gerçekleştirilmesinde intrakorporeal bir kez düğümleme imkanı sağlayan kontinü sütür tekniği güvenilir olmasının yanında sadece anastomoz süresinde değil, aynı zamanda ameliyat ve üretral kataterizasyon süresinde anlamlı oranda kısalma sağlamaktadır.
