Sterilisation of hydrophylic catheters used for intermittent catheterisation in domestic microwave oven

Abstract
Clean intermittent catheterization (CIC) is an effective method in the treatment of patients with neurogenic voiding dysfunction. To reuse the pre-lubricated, hydrophilic catheter which is used for this purpose in sterile conditions, we evaluated the sterilisation in domestic microwave oven and its effect on catheter integrity. Sterile 8F Easicath® catheter was initially incubated with common urinary pathogens such as E.coli, Enterococcus, S. aureus, Pseudomonas and Enterobacter. All catheters showed positive cultures. Then 3 groups each containing 10 catheters were formed and each group was manipulated by different procedure. Also each group had 2 subgroups where "a" catheters were left wet and "b" was dried. Gr-I Mechanical cleaning of the catheters only with soap and water, only Gr-II : Without mechanical cleaning, catheters were placed directly in the 700 watt microwave oven for 30, 60, 90 seconds, Gr-IIa wet and Gr-IIb dried Gr-III After mechanical cleaning with soap and water microwave oven sterilisation with the same power and time intervals was carried out. Gr-IIIa wet and Gr-IIIb dried. Cultures were taken from each catheter following these procedures. In the two subgroups cleaned by soap and water only (+) cultures were obtained. Melting began especially in the wet catheters from 60th second and over and (+) cultures were obtained in all. In the dry catheter subgroup, in the 60th second of microwave oven, growth was observed where, as no growth could be detected in 60th second of the mechanical cleaning plus microwave sterilisation group. Repeat sterilisation in 60th second did not affect the integrity of dry catheters. Catheter body showed no difference but the distal end of the-catheter were significantly melted at the 90th second. It was determined that sterilisation of pre-lubricated, hydrophilic catheters by a 700 watt domestic microwave oven for 60 seconds after mechanical cleaning is effective in sterilisation and does not damage the structure of the catheter. This provides an aseptic and economical method of sterilising CIC's.

Temiz aralıklı kateterizasyon (TAK) nörojenik işeme bozukluğu olan hastaların tedavisinde etkili bir yöntemdir. Bu amaçla kullanılan kayganlaştırılmış, hidrofilik kateterlerin steril şartlarda bir çok kez kullanılmasını sağlamak üzere ev tipi mikrodalga fırının sterilizasyona ve kateter yapısına etkisi araştırıldı. Steril 8F Easicath® kateterler öncelikle üriner enfeksiyonlu hastalardan elde edilen E.coli, Enterococcus, S. aureus, Pseudomonas ve Klebsiella suşlarıyla enkübe edildi. Tüm kateterlerden kültür alınarak enfekte oldukları kanıtlandı. Daha sonra kateterler 3 ana gruba ayrıldı ve 10 kateterlik alt gruplar halinde farklı işlemlere tabi tutuldu. Gr-I: Sadece sabunlu su ile mekanik temizlik yapılıp kateterler ıslak iken (Gr-Ia) ve kurutulduktan sonra (Gr-Ib) kültür alındı. Gr-II: Mekanik temizlik yapılmadan sadece mikrodalga fırında 700 watt şiddette 30, 60, 90 sn sürelerle ıslak (Gr-IIa) ve kuru (Gr-IIb) olarak tutulup daha sonra kültür alındı. Gr-III: Mekanik temizliği takiben mikrodalga fırında aynı şiddet ve sürelerde ıslak (Gr-IIIa) ve kuru (Gr-IIIb) olarak tutulup sonra kültür alındı. Kanlı ağar besi yerine yapılan ekimler 24-48 saat sonra üreme açısından kontrol edildi. Sadece sabunlu su ile mekanik temizlik yapılan iki grupta da üreme gözlendi. Özellikle kateterler ıslak durumdayken mikrodalga fırına konulan kateterlerde 60. saniyeden itibaren erime başlarken, hepsinde üreme gözlendi. Kuru olan grupta ise sadece mikrodalga uygulanan kateterlerde 60. saniyede üreme gözlenirken, mekanik temizlik + mikrodalga uygulanan grubun 60.saniyesinde üreme gözlenmedi. Aynı süre ile tekrarlanan sterilizasyon seanslarının da kateter yapısını etkilemediği görüldü. 90 saniyede ise kateterlerin proksimal uç ve gövde kısmında değişiklik olmazken, distal uçları belirgin olarak eridi. TAK için kullanılan kayganlaştırılmış hidrofilik kateterlerin mekanik temizliği takiben ev tipi mikrodalga fırında 700 watt 60 sn süreyle sterilizasyonunun efektif olduğu ve kateter yapısını bozmadığı görülmüştür. Bu yöntemle, TAK'un hem ekonomik hem de aseptik şartlarda yapılması mümkün olacaktır.
