Comparison of the meatal-based flap (Mathieu) and the tubularized incised-plate uretroplasty (Snodgrass: Tipu) in distal hypospadias repair

Abstract

In this study, we aimed to compare Mathieu (Meatal based flap urethroplasty) and Snodgras:TIPU (Tubularized incised-plate urethroplasty) methods in distal hypospadias repair. Between the January 1996 and April 2001 sixty-five patients with distal hypospadias were operated on. The ages of the patients ranged between the 2 and 27 years (mean 18 year). Mathieu method was used in 28 patient and TIPU method was used in 37 patients. All of the patients were primary case and 23 of them were circumcised. Fooley catheter was used for the urinary diversion methods in all patients. Catheters were removed 10 to 12 days after operation. Fistula was developed in 4 (14.8%) patients undergoing Mathieu repair. In the TIPU repair group, 4 patients (8.1%) (p<0.05) developed uretral fistula and one patients (2.7%) developed both urteral fistula and meatal stenosis. All of the uretral fistulas developed in adult patients in both groups. The duration of surgery was between 60 -90 min (mean 75 min) for the Mathieu repair and was between 40-70 min. (mean 50 min) (p<0.05) for the Snodgrass repair. Our treatment method for the distal hypospadias repair was changed in favor of TIPU repair for two years because of quicker operation time, lower complication rate and easier method.

Distal (koronal ve subkoronal) hipospadiyasların onarımında en sık tercih edilen yöntemlerden ikisi olan MATHIEU (Meatal tabanlı flep üretroplasti) ve SNODGRASS: TIPU (Tübülarize insize plak üretroplasti) yöntemlerini karşılaştırdık. Ocak 1996-Nisan 2001 tarihleri arasında yaşları 2 ile 27 (Ortalama 18) arasında değişen 65 koronal ve subkoronal hipospadiyaslı hasta opere edildi. 28 hastada Mathieu, 37 hastada TIPU yöntemleri tercih edildi. Tüm hastalar primer bunların 23'üde sünnetli idi. Diversiyon tekniği olarak silikon Foley sonda kullanıldı. Sonda 10-12. günlerde çekildi. Mathieu tekniği ile opere edilen 4 hastada (% 14.8) fistül gelişti. TIPU uygulanan grupta ise 3 hastada (%8.1) fistül (p<0.05), 1 hastada ise (%2,7) fistülle birlikte meatal darlık gelişti. Fistüllerin tamamı her iki teknikte de erişkin yaş grubu hastalarda gelişti. Mathieu uygulanan grupta operasyon zamanı 60-90 dakika (Ortalama 75 dakika), TIPU uygulanan grupta 40-70 dakika (Ortalama 50 dakika) (p<0.05) oldu. Daha kısa operasyon zamanı, daha düşük komplikasyon oranı, kolay uygulanımı nedeni ile özellikle üretral oluğu belirgin olan distal hipospadiyas hastalarında son iki yıldan beri tercihimiz TIPU yöntemi lehine değişmiştir.
