Laparoscopic extraperitoneal cystourethropexy: Our results in 72 patients

Abstract

Cystourethropexy has been regarded the treatment of choice and well applied in the surgical management of Stress Urinary Incontinence (SUI). On the other hand, laparoscopic cystourethropexy has gained popularity in the treatment of SUI with/without low-grade cystocele. We evaluated the long-term results of Laparoscopic Extraperitoneal Cystourethropexy (LEC) in selected 72 patients. From 1996 to 2001, LEC has been applied to 72 patients with SUI with/without low-grade cystocele and have been followed for a minimum 4 years period. Following a detailed anamnesis, physical examination (including pad and Boney test) has been performed and symptom score of SEAPI was obtained. In case of an uneventful surgery, patients were seen regularly until the end of fourth years. Results were analyzed statistically with Mann Whitney-U test. While the symptom score of the SEAPI were found 13±2 subjectively and 11±3 objectively in the preoperative term at the end of the four year follow-up, it was found to be 3.5±0.6 subjectively and 2±1.3 objectively. Complete cure rate was achieved in 69 patients (96%) at the end of first year and 55 (78%) at the end of four years. Urgency symptoms were found in 47% of the patients at first evaluation and leukoplakia of the bladder on cystoscopic examination has been detected in nearly half of them. Conclusion: Our results has shown that; laparoscopic cystourethropexy was found to be well enough from surgical standpoint and it has been well tolerated by the patients during postoperative period with effective complete cure results in short as well as long-term follow-up. We believe that, shorter operation time and acceptable hospital staying with minor degree of wound scar formation will make this technique popular and a good alternative in near future.

Stres üriner inkontinans (SÜI) tedavisinde, sistoüretropeksi operasyonları erken dönemde oldukça başarılı sonuçlar ortaya koyarken, uzun dönemde başarı oranlan dramatik şekilde azalmaktadır. Laparoskopik ekstraperitoneal sistoüretropeksi (LES), özellikle düşük dereceli sistoselin eşlik ettiği stres üriner inkontinans olgularında, ortaya koyduğu başarılı sonuçlarıyla dikkat çekmektedir. Kliniğimizde seçilmiş 72 olguda uygulanan LES operasyonun uzun dönem sonuçlarını değerlendirdik. 1996-2001 tarihleri arasında 72 stres üriner inkontinans olgusuna LES uygulandı. Olgular detaylı anamnez, fizik muayene (pad ve boney testi içeren) ve SEAPI semptom skoru ile değerlendirildi. Postoperatuar olarak anormal gelişme olmadığı takdirde SEAPI skoru 1. ve 4. yılda tekrarlandı. Sonuçlar Man Whitney-U testi ile istatistiksel anlamlılık yönünden değerlendirildi. Operasyondan önce semptom skoru sübjektif olarak 13±2, objektif olarak 11±3 bulundu. Operasyon sonrası değerler istatistiksel anlamlı farklılık göstermiş ve sübjektif olarak 3.5+0.6, objektif olarak ise 2±1.3 olarak saptanmıştır (p<0.005). Tam başarı oranı birinci yıl sonunda %96 olarak saptanırken, dördüncü yıl sonunda bu oran %78 bulundu. Urge inkontinansı operasyondan önce %47 olguda saptanmış olup, bunların yarısında (n:17) mesanede lökoplaki saptandı. Elde ettiğimiz sonuçlar, LES uygulamasının, tedavi etkinliği ve postoperatif hasta memnuniyetinin yanı sıra kısa ve uzun dönem başarılı sonuçlarıyla etkili olduğunu göstermiştir. Postoperatif skar oluşumunun az olması, operasyon ve postoperatuar hastanede kalış süresinin kısa olması nedeniyle, yöntemin seçilmiş olgularda stres üriner inkontinansı tedavisinde yakın gelecekte daha fazla uygulanacağına inanıyoruz.
