Transurethral needle ablation (TUNA) in treatment of benign prostatic hyperplasia

Abstract

Although transurethral resection of prostate (TUR-P) is considered as a "gold standard" for the surgical treatment of benign prostatic hyperplasia (BPH), the morbidity rate of this procedure is still about 3-18% so, the current trend is toward minimal invasive therapies. We evaluated the efficacy and safety of TUNA that a new minimal invasive procedure for the treatment of BPH. 23 patients who age above 40 years, no suspicion of prostate cancer according to clinical or laboratory findings, the prostate volume less than 70 grams, maximum urinary flow rate (Qmax) less than 15 ml/s and International Prostate Symptom Score (IPSS) greater than 13 were included the study. The mean age of patients was 61.2 years (range 55 to 71) and the mean prostate volume was 44.8 (33-70) grams. TUNA procedure was performed under epidural anesthesia. The mean operation time was 42 min. After the procedure, an urethra! catheter was inserted to all patients and removed on first day. At the end of 6 months, Qmax, IPSS and Quality of Life (QOL) parameters were compared with preoperative values. The results were assessed by Wilcoxon signed rank test. At 6-months follow-up, statistically significant improvement was obtained for Qmax, IPSS and QOL parameters (P=0.016, P=0.017, and P=0.014 respectively). Only one patient could not urinate and a urethra! catheter was left for four days. After procedure, most of the patients had suffered from irritative symptoms for five to seven days. Mild temporary hematuria was observed in a few patients for two to three days. No patient required an additional procedure. Erectile dysfunction, incontinence or retrograde ejaculation was not seen. Significant improvement was observed in Qmax, IPSS, and QOL parameters, 66.3%, 62.1%, and 62.5% respectively. Although our follow-up period and patient number are limited, TUNA seems to be an effective, safe and easily applicable treatment method of BPH with a fairly low side effect. TUNA should be considered an alternative of TUR-P in patients with BPH who want to protect own sexual functions

Benin prostat hiperplazisinin (BPH) cerrahi tedavisinde transüretral prostat rezeksiyonu "altın standart" olarak kabul edilmesine rağmen bu tedavinin morbiditesi hala %3-18 civarındadır. Bu nedenle günümüzde minimal invaziv yöntemlere eğilim artmıştır. Bu çalışmada yeni bir minimal invaziv yöntem olan TUNA'nın, BPH tedavisinde etkinlik ve güvenirliliği değerlendirilmiştir. 40 yaşından büyük, klinik ve laboratuar olarak prostat kanseri şüphesi olmayan, prostat ağırlığı 70 gramdan ve maksimum akım hızı (Qmax) 15 ml/sn.den düşük olan ve Uluslararası Prostat Semptom Skoru (IPSS) 13'ten fazla olan 23 hasta çalışmaya alındı. Hastaların yaş ortalaması 61.2 yıl (55-71) ve ortalama prostat hacmi 44.8 (33-70) gram idi. TUNA işlemi epidural anestezi altında yapıldı. Ortalama işlem süresi 42 dakika idi. İşlem sonrasında hastaların hepsine üretral kateter konuldu ve birinci gün çıkarıldı. Pre-op Qmax, IPSS ve Quality of Life (QOL) Index parametreleri post-op 6. ay değerleri ile karşılaştırıldı. Sonuçlar Wilcoxon Signed Rank testi ile analiz edildi. 6 aylık takipte, Qmax, IPSS ve QOL parametrelerinde istatistiksel olarak anlamlı düzelme saptandı (sırasıyla P=0.016, P=0.017 ve P=0.014). Sadece bir hasta idrar yapamadığı için dört gün süren kateterizasyon gerekti. İşlemden sonra hastaların çoğunda 5-7 gün süren irritatif semptomlar ve bazı hastada 2-3 gün süren geçici hematüri gözlendi. Hastaların hiçbirine ek bir müdahale gerekmedi. Erektil disfonksiyon, inkontinans ve retrograt ejakülasyon görülmedi. Qmax, IPSS ve QOL parametrelerinde sırasıyla, %66.3, %62.1 ve %62.5 oranında, istatistiksel olarak anlamlı düzeyde düzelme tespit edildi. Hasta sayımız ve takip süremiz kısıtlı olmakla beraber TUNA, BPH tedavisi için etkin ve emniyetli, uygulaması kolay ve yan etki oranı oldukça düşük olan bir tedavi yöntemi olarak görünmektedir. TUNA, seksüel fonksiyonlarını korumak isteyen genç BPH'lı hastalar için TUR-P'ye alternatif olarak göz önünde bulundurulabilir.
