The prognostic value of PSA, biopsy Gleason score and clinical stage in patients treated with radical prostatectomy

Abstract
Radical prostatectomy is the first choice treatment in organ confined prostate cancer with the advantage of ultimate cure. Almost half of cases have extraprostatic disease at pathologic evaluation and long-term follow up of 40% of cases display biochemical recurrence. The purpose of our study was to determine the relation between preoperative PSA, biopsy Gleason scores and the clinical stage with the recurrence of the disease after surgery. We have retrospectively evaluated 70 cases having undergone radical prostatectomy in our clinic between 1991 and 2000. The progression free survival rate was 73% and overall survival rate was 93% at a median follow up of 57 months. The mean preoperative PSA level was 17.8 ng/ml. There were 12 cases with a clinical stage of T1, 51 with T2a and 7 cases with T2b. There were 28, 34 and 8 cases for preoperative biopsy Gleason scores of 2-4, 5-6 and 7-10 respectively. The PSA progression rate was 14%, 35% and 50% in cases with preoperative biopsy Gleason scores of 2-4, 5-6 and 7-10 respectively. The progression rate was 25% in 12 cases with cT1 27 % in 51 cases with cT2a and 28 % in 7 cases with cT2b. The progression rates of the cases with preoperative PSA values of 0-9.9 ng/ml, 10-19.9 ng/ml and above 20 ng/ml were 5%, 26% and 48% respectively. Clinical stage was not correlated with the progression rate, but the preoperative PSA level was found to be the best prognostic factor. Although a 43% downgrading was detected when comparing biopsy and specimen Gleason scores, these scores were still found to be correlated with the progression rates.

Organa sınırlı prostat kanserli olgularda radikal prostatektomi, hastayı tamamen kansersiz bırakma avantajı nedeniyle ilk seçilen tedavi şeklidir. Ancak cerrahi sonrasında olguların yarıya yakınında ekstraprostatik yayılım ve uzun takiplerde %40'lara varan nüks oranları bildirilmektedir. Çalışmanın amacı klinik evre, preoperatif PSA düzeyi ve ameliyat öncesi biyopsi Gleason skorunun nüks ile olan ilişkisinin saptanması ve prognozda risk faktörü olarak değerinin araştırılmasıdır. Bu amaçla 1991-2000 yılları arasında yaş ortalaması 64 olan toplam 70 radikal prostatektomili olgu araştırıldı. Ortalama 57 ay takip edilen olgularda sağ kalım oranı %93 (65/70) ve hastalıksız sağ kalım oranı %73 (51/70) olarak tespit edildi. Olguların preoperatif PSA ortalamaları 17.8 ng/ml idi. Hastalar klinik evrelerine göre T1: 12 olgu, T2a: 51 olgu ve T2b: 7 olgu idiler (2000 TNM sınıflamasına göre). Ameliyat öncesi biyopsi Gleason skorlarına göre 2-4 arası 28, 5-6 arası 34 ve 7-10 arası 8 olgu mevcut idi. Ameliyat öncesi biyopsi Gleason skoru 2-4 arası olgularda %14, 5-6 arası olgularda %35 ve 7-10 arası olgularda %50 oranında PSA progresyonu saptandı. Klinik evrelerine göre cT1 12 olgu %25 oranında, cT2a 51 olgu %27 oranında ve cT2b 7 olgu %28 oranında progresyon gösterdi. Olgular ameliyat öncesi PSA değerlerine göre değerlendirildiğinde 0-9.9 ng/ml arası olguların %5'i, 10-19.9 ng/ml arası olguların %26'sı ve 20 ng/ml ve üzeri olan olguların %48'inin progresyon gösterdiği saptandı. Sonuç olarak ameliyat öncesi PSA değerinin hastalığın nüksü ile yakın ilişkisi nedeniyle en iyi prognostik faktör olduğu, biyopsi Gleason skoru ile rekürrensin yakın ilişkili olduğu ancak %43 oranında düşük derecelendirme yapıldığı ve cerrahi tedavi uygulanan olgularda klinik evrenin rekürrens ile ilişkili olmadığı sonucuna varıldı.
