Results of nesbit operation in surgical treatment of penile curvatures

Abstract
Introduction: Penile deviation is a common problem in the urological practice. Nesbit operation is a simple and safe method in correction of penile deformity due to congenital or acquired curvatures. In this study, we evaluated surgical results of Nesbit procedure in 17 patients with penile curvature of different etiologies. Material and methods: 17 patients who treated for penile curvature by Nesbit technique were examined in between 1996-2001. Mean age value was 41 (range between 16 and 62). In etiology, 4 patients had congenital penile deviation and 1 patient had congenital lack of corpus spongiosum. Acquired deformities were due to Peyronie disease in 8 and trauma in 4 patients. Eight patients with possible Peyronie disease were controlled by penile USG and diameter of plaques was determined. Patients with impotence were not included to the study. Intracavernosal papaverine test was done to control the degree of curvature. Degree of curvature was changed between 30-90°. All patients were operated with spinal anesthesia. There was no complication related to surgery and corrected erection controlled by artificial erection after the procedure. Results: Surgical results were classified as excellent, satisfactory and worse. Overall success rate was %88 with excellent results in 12 patients (%70) and satisfactory in 3 (%18). In one patient with Peyronie disease, surgery was not successful. Second operation with a cutaneous flap was needed in patient with congenital lack of corpus spongiosum which post-operative urethral fistula developed. In 4 patients penile length was shortened less than 2 cm, but did not prevent penetration. Conclusion: Nesbit operation remains the procedure of choice in the correction of penile curvature. Patients should be questioned for sexual function before operation and informed for penile shortening in some cases.

Penil deviasyon üroloji pratiğinde sık karşılaşılan bir sorundur. Nesbit operasyonu doğumsal veya edinsel kurvatürlere bağlı penil deforamitenin düzeltilmesinde basit ve güvenilir bir yöntemdir. Bu çalışmada çeşitli nedenlere bağlı gelişen 17 penil kurvatür olgusunda Nesbit yöntemi sonuçlarını değerlendirdik. 1996-2001 yıllan arasında kliniğimizde penil kurvatür nedeniyle Nesbit yöntemi uygulanan 17 hasta incelendi. Hastalann yaş ortalaması 41 (16-62) idi. Etyolojide 4 hastada doğumsal deviasyon ve l hastada doğumsal spongioz cisim yokluğu saptandı. Edinsel deformitelerin 8'i Peyronie hastalığı, 4'ü ise travmaya bağlı idi. Peyronie hastalığı düşünülen 8 hasta penil USG ile kontrol edildi ve plak büyüklüğü tespit edildi. Çalışmaya impotans yakınmaları olan hastalar dahil edilmedi. Hastaların operasyon öncesi eğrilik dereceleri intrakaveraozal papaverin testi ile kontrol edildi. Eğrilik dereceleri 30-90° arasında değişmekte idi. Hastaların tümüne spinal anestezi ile müdahale edildi. Operasyon esnasında herhangi bir komplikasyon gelişmedi ve işlem tamamlandıktan sonra artifisiyel ereksiyon sağlanarak düzeltilen ereksiyon kontrol edildi. Cerrahi sonuçlan mükemmel, tatminkar veya kötü olarak sımflandmldı. Cerrahi basan oranı 12 hastada (%70) mükemmel, 3 hastada (%18) tatminkar olmak üzere toplam %88 bulundu. Bir Peyronie hastasında sonuç başarısızdı. Doğumsal spongioz cisim yokluğu mevcut l hastada post-op fistül gelişti ve ikinci bir operasyonla flep çevrilerek üretral açıklık kapatıldı. 4 hastada penis boyunda 2 cm'den az bir kısalık oluştu, ancak bu durum penetrasyona engel değildi. Nesbit operasyonu, penil kurvatürlerin tedavisinde hala seçkin bir yöntemdir. Hastalar cinsel fonksiyon yönünden operasyon öncesi mutlaka sorgulanmalı ve bazı olgularda karşılaşılan penis boyunda kısalma yönünden mutlaka bilgilendirilmelidir.
