Treatment of ureter stones with extracorporeal shock waves using a second generation lithotriptor: A retrospective study

Abstract
Introduction: The treatment results of ureteric stones by Multimed 9200 U-armed, a second generation Extracorporeal Shock Wave Lithotripsy (ESWL) device were reviewed retrospectively. Materials and Methods: ESWL was performed in 267 consecutive patients (70 female and 197 male) with a mean age of 36.68±11.23 (range 16-70) years at urology clinics of Elazığ Public Hospital between April 2003 and June 2005. The stones were opaque and/or semiopaque and the procedure was done under flouroscopy. The average stone size of 267 patients was 65.10±33.49 mm<sup>2</sup> and ESWL was performed maximum three times using 16kw (12-18kw) energy and 2250-2500 shock waves according to the localization of the stone. Results: Stones fragmented and expulsed exactly were classified as stone free. Nonsymptomatic fragments 4 mm and less were considered as clinically insignificant residual fragments and stones not expulsed were named as residual stones. The success rate was evaluated by IVP and USG three months after ESWL. During follow-up, 90.2% patients were rendered stone free, 0.7% had clinically insignificant residual fragments and 8.9% had residual stones. Stone free rates for upper (93.5%) and lower (91.8%) ureter were higher than mid ureter. Residual stones were observed more commonly in stones greater than 116 mm<sup>2</sup>, while stone free rates were higher in stones smaller than 60 mm<sup>2</sup>. Conclusion: ESWL is an effective treatment alternative in ureteral stones.

Nisan 2003 ile Haziran 2005 tarihleri arasında Elazığ Devlet Hastanesi Üroloji kliniğinde izlenen 267 üreter taşlı hastanın Multimed 9200 U-kollu, ikinci kuşak ESWL cihazı ile tedavi sonuçları retrospektif olarak gözden geçirildi. Ortalama yaşı 36.68±11.23 (16-70) yıl olan 70'i kadın, 197'si erkek 267 üreter taşlı hastaya ESWL uygulandı. Taşların tamamı opak ve/veya semiopak olup işlem floroskopi yardımıyla yapıldı. 267 hastanın ortalama taş boyutu 65.10±33.49 mm 2 olup her hasta için taşın yerleşimine göre ortalama 2250 ile 2500 arası değişen şok dalga ve ortalama 16kw (12-18kw) enerji ile maksimum 3 seans ESWL uygulandı. Taşın parçalanıp tamamen dökülmesi taşsızlık (stone free), 4 mm ve altı nonsemptomatik parçacıklar klinik önemsiz ve taştan temizlenememe durumu ise artık taş olarak yorumlandı. ESWL'den 3 ay sonra ultrasonografi ve intravenözpiyelografi ile başarı değerlendirildi. Hastaların %90.2'de taşsızlık, %0.7'de klinik önemsiz parçacıklar, %8.9'da ise artık taş tespit edildi. Üst ve alt üreter taşlarında taşsızlık oranı en yüksek iken (%93.5 ile %91.8), orta üreter taşlarında taşsızlık oranı %56.3 olarak bulundu. Taş yüzey alanı 60 mm 2 'den küçük taşlarda taşsızlık oranı en yüksek iken 116 mm 2 'nin üzerindeki taşlarda artık taş kalma oranı yüksek bulundu. ESWL noninvaziv ve ayaktan uygulanabilme kolaylığından dolayı üreter taşı tedavisinde alternatif ve etkin bir tedavi yöntemidir.
