Ureteroscopy and pneumatic lithotripsy: 1056 cases

Abstract

Introduction: In this study, we aimed to assess the treatment outcomes after ureteroscopy (URS) and pneumatic lithotripsy for ureteric stones especially in relation to stone localization and size in an attempt to analyze the efficacy of pneumatic lithotripsy. Short term and long term complications are also presented. Materials and Methods: Data of 1056 patients, who underwent URS and pneumotic lithotripsy for ureteric stones between August 1987 and June 2004, were retrospectively analyzed. Patients were stratified into two different groups with respect to stone localization and size. Groups were compared regarding stone free rate (SFR) and complications. Groups were also assessed for JJ requirement and unintentional stone push-back. Long term complication rates were presented. Results: Overall SFR and major complication rates were 89.8% and 2.9%, respectively. There was a statistically significant difference regarding SFR and complication rates among the three groups with respect to stone localization (p=0.01). Highest SFR (92.9%) and lowest complication rate (2%) were achieved for the distal ureter. SFR decreased while complication rate was increasing with increasing stone size in all ureteric locations. JJ requirement was the highest for the proximal ureter. There was no difference in long term complication rates among the three groups. Conclusion: The outcomes for URS and pnematic lithotripsy yield high success rates and low complication rates specifically in distal ureteric stones. However, SFR decreases while complication rate increases towards the proximal ureter. There is a negative correlation with SFR and a positive correlation with complication rate with increasing stone size.

Bu çalışmamızda üreteroskopi (URS) ve pnömotik litotripsi ile tedavi ettiğimiz üreter taşlarındaki sonuçlarımızı özellikle taş yerleşimini ve büyüklüğü açısından sunmak, kısa ve uzun dönem istenmeyen yan etki oranlarını belirlemek ve pnömotik litotripsinin etkinliğini saptamayı amaçladık. Ağustrs 1987 ve Haziran 2004 tarihleri arasında kliniğimizde üreter taşı nedeniyle tedavi amacıyla URS ve pnömotik litotripsi uygulanmış 1056 hastanın verileri retrospektif olarak incelendi. Taş yerleşimi ve boyutuna göre oluşturulan gruplarda başarı ve istenmeyen yan etki oranları değerlendirildi. Gruplar ayrıca taşın böbreğe geri kaçma oranlan ve ameliyat sırasında JJ gereksinimi açısından karşılaştırıldı. Kayıtlı uzun dönem istenmeyen yan etkileri sunuldu. URS ve pnömotik litotripsi sonrasında %89.8 taşsızlık ve %2.9 önemli istenmeyen yan etki oranı izlendi. Yerleşimine göre oluşturulan gruplar arasında taşsızlık ve istenmeyen yan etki oranlan açısından istatistiksel anlamlı farklılık saptandı (p=0.01). En yüksek başarı (%92.9) ve en düşük istenmeyen yan etki oranı (%2)'alt üreterde izlendi. Artan taş boyutu ile her üç yerleşimde de başarı oranlarının düştüğü, istenmeyen yan etki oranlarının ise arttığı görüldü. JJ gereksiniminin en çok üst üreter taşlarında olduğu izlendi. Uzun dönem istenmeyen yan etki oranları açısından gruplar arasında fark saptanmadı. URS ve pnömotik litctripsi ile özellikle alt üreter taşlarının tedavisinde yüksek başarı ve düşük istenmeyen yan etki oranlarına ulaşmak mümkündür. Ancak üst üretere yaklaştıkça başarı düşmekte, istenmeyen yan etki oranları artmaktadır. Artan taş boyutu ile taşsızlık arasında negatif, istenmeyen yan etki oranı ile ise pozitif korelasyon vardır.
