Outcome of penile prosthesis implantation for treating erectile dysfunction: experience with 335 cases

Abstract
Introduction: The aim of this paper is to present the outcome of 335 penile prosthesis implantation procedures due to organic erectile dysfunction. Materials and Methods: Three hundred and thirty five penile implantation procedures were done in our clinic with the diagnosis of erectile dysfunction between May 1990 and December 2006. Of the prosthesis inserted, 140 were AMS Dynaflex, 153 were AMS Malleable, 15 were Mentor Malleable, 15 were AMS Ambicore, 10 were AMS Ultrex and 2 were Mentor Excell prostheses. The mean follow-up was 71 (3-143) months. Results: The complications after the implantation were infection 2% (n=7), mechanical failure 6% (n=31: 28 Dynaflex, 1 AMS Malleable, 1 Ambicor, 1 Ultrex) and dislocation 1% (n=5, 4 AMS Malleable, 1 Dynaflex). dissatisfaction rate of the patient due to misuse of the pump was 12% (n=20) and decreased to 1% (n=2) after intensive education. Overall complication rate was 13% (n=45). Conclusion: The rates of the complication and dissatisfaction of the patients are acceptable as in the literature. Appropriate patient choice, preoperative preparation and the education of the patient with the correct implant are the most important factors to obtain better results.

Bu çalışmamızda, organik erektil disfonksiyon tanısı ile penil protez yerleştirilen 335 hastalık serimizde istenmeyen yan etki oranlarımız ve deneyimlerimizi sunmayı amaçladık. Mayıs 1990 ile Aralık 2006 arasında organik erektil disfonksiyon tanısı konan 335 hastaya penil protez yerleştirilmiştir. Yerleştirilen protezlerin dağılımı; 140 AMS Dynaflex, 153 AMS Malleable, 15 Mentor Malleable, 15 AMS Ambicor, 10 AMS Ultrex, 2 Mentor Excell şeklindedir. Hastaların izlem süresi ortalama 71 (3-143) ay idi. Penil protez yerleştirilmesi sonrası görülen spesifik istenmeyen yan etkiler ve oranlarımız şu şekildedir: protez enfeksiyonu: %2 (n=7), mekanik olarak cihaz hasarı: %6 (n=31: 28 Dynaflex, 1 AMS Malleable, 1 Ambicor, 1 Ultrex), dislokasyon: %1.49 (n=5, 4 AMS Malleable, 1 Dynaflex), pompayı kullanamama nedeniyle hasta memnuniyetsizliği: %12.04 (n=20) (iken yoğun eğitimden sonra bu oran %1.2 (n=2) oldu). Genel olarak tüm istenmeyen yan etki oranımız %13.4 (n=45) olarak bulundu. Klinik deneyimlerimizde penil protezlerin istenmeyen yan etki oranları ve hasta memnuniyetsizliği kabul edilebilir olarak bulunmuştur. Daha iyi sonuçlar elde edebilmek için uygun hasta seçimi, ameliyat öncesi doğru hasta hazırlığı, yerleştirilecek penil protez hakkında yeterli hasta bilgilendirilmesi ve uygun teknik kullanılmasının önemli olduğu bilinmelidir.
