Are patients admitted to urology clinic aware of smoking being a risk factor for bladder cancer?

Abstract

Smoking is one of the most recognized risk factors leading to the development of bladder cancer. The duration and the quantity of cigarettes smoked are proportional to the risk of cancer development which continues for many years even after quitting it. In this study, we prompted to evaluate the awareness of the patients referred to urology clinic for any purpose on the role of smoking in the etiology of bladder cancer. Materials and Methods: A total of 461 patients were included into the study. Age, sex, monthly income, educational status and smoking habits in addition to their awareness about the relation between smoking and bladder, kidney and lung cancer were noted and correlated with sex, educational status and monthly income levels. Results: There were 338 (73.3%) males and 123 (26.7%) females. The mean age was 48.91±15.22 (18-87). Of these, 139 patients (30.15%) were smoking, 103 patients (22.34%) were ex-smokers and 219 (47.51%) never smoked before. One hundred-fourteen (82%) of the smokers were male. There was a statistically significant difference in smoking habits between males and females (p=0.01). The ratio of patients who are aware of the link between smoking and bladder cancer was 35.6%. On the other hand, 31.9% and 77.7% of patients were aware of the association between smoking and kidney and lung cancers, respectively. The majority of the patients included into the study and smokers were primary school graduates. There was a statistically significant relation between smoking habit and the educational status of the patients (p=0.029), amongst the high school and the college graduates the incidence of smoking was the highest. No correlation between monthly income levels and smoking was observed (p=0.686). College graduates and master’s degree and Ph.D. holders were aware of the relation between smoking and all three types of cancer development (p=0.018 for bladder cancer, p=0.02 for kidney cancer and p=0.001 for lung cancer). There was no statistically significant relation between the awareness of the association of smoking and bladder cancer and monthly income (p=0.054). On the other hand association between the awareness of smoking and development of kidney and lung cancer and monthly income levels was significant (p=0.024, p=0.002 respectively). There was no statistically significant association between genders in the awareness of the role of smoking in the etiology of lung, bladder and kidney cancer (p=0.253, p=0.238, p=0.868 respectively). Conclusion: The awareness of smoking as the causative etiologic factor in lung cancer development was significantly higher in all monthly income and educational status levels compared to those between smoking and bladder or kidney cancer development. Approximately 1/3 of the patients were aware of smoking as a risk factor in bladder cancer development. The theme of “smoking causes bladder cancer” should urgently and relentlessly be delivered to public regardless of monthly income and educational status levels.

Bu çalışmada üroloji polikliniğine başvuran hastaların, sigaranın mesane kanseri etiyolojisindeki rolünü bilip bilmedikleri araştırıldı. Polikliniğimize herhangi bir sebeple başvuran 461 hasta çalışmaya alındı. Hastalara yaş, cinsiyet, gelir ve eğitim düzeyi, sigara alışkanlıkları ile sigaranın mesane, böbrek ve akciğer kanseri açısından risk faktörü olduğunu bilip bilmedikleri soruldu. Elde edilen cevapların cinsiyet, eğitim ve gelir düzeyi açısından farklılık gösterip göstermediği araştırıldı. Hastaların 338’i (%73.3) erkek, 123’ü (%26.7) kadındı. Yaş ortalaması 48.91±15.22 idi. Toplam 139 hasta (%30.15) halen sigara kullanıyordu. Sigara kullananların 114’ü (%82) erkek, 25’i kadın (%18) idi. Erkek ve kadınlar arasında sigara alışkanlığı bakımından istatistiksel olarak anlamlı fark vardı (p=0.01). Sigaranın mesane, böbrek ve akciğer kanseri için risk etkeni olduğunu bilenlerin oranı sırasıyla %35.6, %31.9 ve %77.7 idi. Lise ve üniversite mezunları en yüksek oranda sigara alışkanlığına sahipti. Eğitim düzeyi ile sigara alışkanlığı arasında istatistiksel olarak anlamlı bir ilişki vardı (p=0.029). Aylık kazanç ile sigara alışkanlığı arasında istatistiksel olarak anlamlı bir ilişki yoktu (p=0.686). Her üç kanser türü için de üniversite ve lisansüstü eğitimi alanlar yüksek oranda sigara bilincine sahipti ve eğitim düzeyi ile sigara bilinci arasında istatistiksel anlamlı bir ilişki vardı (mesane: p=0.018, böbrek: p=0.02, akciğer: p=0.001). Sigaranın mesane kanseri için risk etkeni olduğunun bilinip bilinmemesi ile gelir düzeyi arasında istatistiksel olarak anlamlı bir ilişki yoktu (p=0.054) ancak bu ilişki böbrek ve akciğer kanserleri için istatistiksel olarak anlamlıydı (sırasıyla p=0.024, p=0.002). Gelir düzeyi yükseldikçe hastaların verdiği doğru cevap sayısı her üç kanser için de artıyordu. Cinsiyet açısından akciğer, mesane ve böbrek tümörleri için sigara sorgulandığında istatistiksel olarak anlamlı bir fark yoktu (sırasıyla p=0.253, p=0.238, p=0.868). Sonuç olarak, tüm gelir ve eğitim düzeylerinde akciğer kanseri için sigara bilinci düzeyi mesane kanserinden belirgin olarak yüksektir. Sigaranın mesane kanseri için risk etkeni olduğu hastaların yaklaşık 1/3 tarafından bilinmektedir. Bu da tüm gelir ve eğitim düzeylerini hedef alan bir bilinçlendirme eğitimine ihtiyaç olduğunu göstermektedir.
