Tension-free vaginal tape (TVT) for the treatment of genuine stress urinary incontinence

Abstract
The short term safety and efficacy of the tension-free vaginal tape (TVT) for treating genuine stress incontinence (GS1) were evaluated. Between October 1999 and February 2001 TVT were performed in 34 patients with a mean age of 50.5 (38-67). Seven patients had previous unsuccessful surgery for stress incontinence. Valsalva leak point pressures were lower than 60 mmHg in 9 patients and 3 patients had detrusor instability. Mean operative time was 37.2 minutes (20-70) and patients were discharged home in a mean of 1.4 days (1-5). Bladder perforations occurred in 2 patients (6%). The cure rates were more than 90% with a mean follow-up of 9.2 months (1-17). Twenty-nine patients (85%) were able to micturate immediately after the procedure. One patient (3%) experienced urinary retention that lasted for more than 4 weeks postoperatively. When the preoperative and postoperative maximal flow rate (30.2 ml/sec and 22.9 ml/sec) in the remaining 33 patients were compared, significant decrease were assessed (p=0.001), although the difference in residual urinary volume was insignificant (p=0.08). Urge incontinence were resolved 66% of patients who complained of mixed incontinence preoperatively. No one had de novo urge symptoms. Four patients (12%) had significant pain that resolved approximately 1 month after the operation. Recurrent localized infection were determined at the suprapubic incision in 1 patient (3%), in another patient prolen fistula was occurred 2 cm below the suprapubic incision. Of the 27 patients, 24 (89%) stated that they were completely dry, would undergo the surgery again and would recommend TVT to a friend. In conclusion, TVT appears to be a safe and effective treatment for GSI in short term. The advantages of the technique are ease in performance and minimal morbidity. Further long term and prospective randomized controlled studies comparing TVT with other techniques are needed.

Gerçek stres inkontinans tedavisinde, tansiyonsuz vajinal teyp'in (TVT) kısa dönem etkinliği ve güvenilirliği araştırıldı. Ekim 1999 ve Ocak 2001 tarihleri arasında gerçek stres inkontinans (GSİ) tanısı konan ve ortalama yası 50.5 (38-67) olan ardışık 34 hastaya tansiyonsuz vajinal teyp (TVT) uygulandı. Yedi hastanın (%21) anamnezinde başarısız stres inkontinans cerrahisi belirlendi. Dokuz hastada (%26) valsalva kaçırma anı basıncı 60 cmH2O altında bulunurken, 3 hastada (%9) detrusor instabilite tespit edildi. Ortalama operasyon süresi 37.4 dk. (20-70), ortalama hastane kalış süresi ise 1.4 gün (1-5) idi. İki hastada (%6) işlem sırasında mesane perforasyonu oluştu. Hastalar ortalama 9.2 ay (1-17) takip edildi ve kür oranları %90'ın üzerinde seyretti. Yirmi dokuz hasta (%85) spontan idrar yaptı. Bir hastada (%3) l aydan uzun süren idrar retansiyonu saptandı. Diğer 33 hastanın preoperatif (30.2 ml/sn) ve postoperatif (22.9 ml/sn) maksimal akım hızlan karşılaştırıldığında anlamlı azalma saptanırken (p=0.001), rezidüel idrar miktarında belirgin fark belirlenemedi (p=0.08). Preoperatif mikst inkontinansı olan hastalardan %66'sında sıkışma semptomları kayboldu ve hiçbir hastada de novo sıkışma semptomları gelişmedi. Dört hasta (%12) yaklaşık l ay süren belirgin ağrı tanımladı. Hastalardan l'inde (%3) suprapubik insizyon yerinde tekrarlayan lokalize iofeksiyon, diğer l hastada ise suprapubik insizyon yerinin 2 cm altında ciltten prolen fistülü saptandı. Sorgulaması yapılan 27 hastadan 24'ü (%89) tam iyi olduklarını, gerekirse aynı operasyonu tekrar olabileceklerini ve yakınlarına tavsiye edeceklerini belirttiler. Sonuç olarak GSİ tedavisinde TVT, erken dönem sonuçlan ile etkin ve güvenli bir yöntem olarak gözükmektedir. Uygulama kolaylığı ve postoperatif morbiditesinin minimal olması tekniğin avantajlarıdır. Ayrıca uzun dönem sonuçlarını bildiren ve diğer yöntemlerle karşılaştıran prospektif randomize çalışmalara gereksinim vardır.
