The significance of urinary fibrin / fibrin degradation products in the diagnosis and follow-up of bladder tumors

Abstract
There is a necessity for the presence of the tumor markers predicting the diagnosis of bladder cancer in patients with the symptom of hematuria and recurrence during the follow-ups of the patients. In the present study, urine specimens of the 22 patients attending to our clinic with the symptom of hematuria and 16 patients who were in the follow-up with diagnosis of superficial bladder cancer were examined to detect the presence of fibrin/fibrinogen degradation products by the Accu-Dx tm test. The accuracy of the test results was checked in all patients by the cystoscopy and biopsy when needed. The test revealed positivity in 12 of the 38 patients. Of these 12 patients, cystoscopy and biopsy revealed false positivity in 4 (33.3%) cases. In four cases (15%), a false negativity was recognized in 26 patients whose test results were negative. In all patients, positive and predictivity values of the test were found as 66.7% and 84.6%, correspondingly.

Hematüri yakınması ile başvuran hastalarda mesane kanseri tanısını ve tanı almış olgularda rekürrensi invazif olmayan yöntemlerle predikte edecek tümör belirleyicilerine ihtiyaç vardır. Bu çalışmada, hematüri yakınması ile başvuran 22 hasta ile yüzeyel mesane kanseri nedeni ile izlemde olan 16 hastanın idrar örnekleri Accu-Dx tümör testi ile fibrin ve fibrin yıkım ürünlerinin varlığı açısından incelendi Tüm olgularda sistoskopi ve gereğinde biyopsi alınarak Accu-Dx tümör testinin doğruluğu kontrol edildi. Accu-Dx tm testi yapılan 38 olgunun 12'sinde test pozitif sonuç verdi. Sistoskopi ve biyopsi sonucunda, test sonucu pozitif olan 12 olgunun 4'ünde (%33.3) yalancı pozitiflik olduğu belirlendi. Accu-Dx testi ile negatif sonuç alınan 26 olgunun 4'ünde (%15) ise yalancı negatiftik olduğu belirlendi Tüm hastalarda Accu-Dx tm testinin pozitif öngörü değeri %66.7 ve negatif öngörü değeri %84.6 olarak bulundu.
