Nephron sparing surgery in renal tumors

Abstract
As the incidental detection of renal tumors in low clinical stage increases, the application of nephron sparing surgery (NSS) has arises more interest Between December 1988 and February 2000 NSS was performed in 40 patients (14 male, 26 female) with a mean age 52.4 (19-82). Elective indication group (Group I) with normal contracteral kidney was composed of 24 patients whereas, there were 16 patients in the imperative indication group (Group D). Incidentally detected renal tumors were 67% in Group I, 50% in Group H, with a total percentage of 60%. In Group I, the mean diameter of the tumor was significantly smaller (p=0.02) and more patient undewent partial nephrectomy compared to enucleation (p=0.05). However, the mean operation time as well as the mean clamping time did not differ significantly in two groups (p>0.05). In the histologic evaluation 7 patients, 6 of which were from Group I had benign tumor (17.5%), whereas the remaining 33 patients had renal cell carcinoma. All the patients with the renal cell carcinoma, except one, had local tumor stage Tl and T2. Total number of 7 patients (17.5%), 6 from Group D, had major complications. Complications were significantly higher in Group n (p=0.01). Out of 33 patient with renal cell carcinoma, 2 died due to unrelated causes other than carcinoma, besides one patient died due to multiple organ metastases despite the absence of local recurrence. One patient was lost to follow up. In the remaining 29 patients with a mean follow up of 41.8 months (6-134) local recurrence or distant metastases were not detected and the values indicating kidney function did not differed compared to preoperative period. Overall and cancer specific survivals were 100 and 100% in Group I, 7İ and 88% in Group ü, 84 and 96% in both groups, respectively. In conclusion, NSS is an effective and reliable treatment in low stage renal tumors. However the patients who underwent NSS with elective indication outcome with better results, compared to those with imperative indication.

Böbrek tümörünün rastlantısal olarak tespitinin artmasıyla birlikte uygun klinik evredeki böbrek tümörlerinde nefron koruyucu cerrahi (NKC) uygulanması da hız kazanmıştır. Kliniğimizde Aralık 1988 ile Şubat 2000 döneminde yaslan 19 ile 82 arasında değişen (ort 52.4) 14'ü kadın 26'sı erkek toplam 40 hastaya NKC uygulandı. Karşı taraf böbreği normal olan 24 hasta elektif endikasyon grubu (Grup I) diğer 16 hasta ise zorunlu endikasyon grubu (Grup II) olarak kabul edildi. Grup I'de % 67, Grup II' de % 50 olmak üzere tüm grubun %60'ını rastlantısal saptanan tümörler oluşturdu. Ortalama tümör çapının Grup I'de anlamlı olarak daha küçük olduğu (p=0.02) ve bu gruptaki hastalarda parsiyel nefrektominin enükleasyona oranla anlamlı olarak daha fazla uygulandığı (p=0.05) belirlendi. Ancak gruplar arasında ortalama operasyon süresi ve klamp süresi açısından anlamlı fark bulunmadı (p>0.05). Histopatolojik incelemede 6'sı Grup I'den toplam 7 hastada (%17.5) benign tümör, diğer 33 hastada ise böbrek hücreli kanser saptandı. Böbrek hücreli kanser saptanan hastaların biri hariç tümünde lokal tümör evresi T1 veya T2 olarak belirlendi. Preoperatif ve erken postoperatif dönemde 6'sı Grup II'den toplam 7 hastada (%17,5) majör komplikasyon tespit edildi Komplikasyonlann Grup II'de anlamlı olarak daha yüksek olduğu (p=0.01) belirlendi. Böbrek hücreli kanser tespit edilen 33 hastadan 2'si kanser dışı nedenlerle, 1 hasta ise lokal nüks saptanmamasına rağmen multipl organ metastazlan nedeniyle kaybedildi. Bir hasta ise takip dışı olarak kabul edildi. Kalan 29 hasta için ortalama 41.8 ay (6-134) takip süresi sonucunda nüks bulunmadığı (lokal veya uzak) ve böbrek fonksiyonlarının preoperatif değerlerinden farksız olduğu saptandı. Tüm ve kansere özgü beş yıllık sağ kalım sırasıyla; Grup 1'de %100 ve %100, Grup II'de %71 ve %88, tüm grupta ise %84 ve 96 olarak bulundu. Sonuç olarak, NKC uygun evredeki böbrek tümörlerinin tedavisinde etkin ve güvenilir bir yöntemdir. Ancak elektif endikasyonla öpere edilen hastalarda zorunlu endikasyonla opere edilenlere nazaran daha iyi sonuçlar elde edilmektedir.
