The abuse and cost of free PSA

Abstract

In this study, we aimed to determine the additional value and cost of adding free PSA to the diagnosis of prostate cancer. The files of 882 patients who had total and free PSA values determined and underwent TRUS were reviewed. For values of 0-3.9 ng/ml, 4-9.9 ng/ml and >10 ng/ml and suspicious DRE finding, the total number cancer cases were compared to the number of cases who had a FPSA ratio <0.15. The cost of determining FPSA per patient was also analyzed. For TPSA value of 0-3.9 ng/ml, biopsying the cases who have a FPSA ratio <0.15 yields a 0.05 cancer detection rate with a cost of over 900 million TL per patient For patients whoa have a TPSA value>10 ng/ml or a suspicious DRE the cancer detection rate is 43.1% and 32.5 respectively, compared to 25.7% and 18.1% for patients with a FPSA ratio <0.15 respectively for the same groups (p=0.01). Adding FPSA to the diagnosis in these group of patients increases the diagnostic cost by 100%. There is no statistical difference in the cancer ratios between the patients who have a TPSA value of 4-9.9 ng/ml and FPSA ratio <0.15. We conclude that FPSA determination should only be reserved to patients who have a TPSA value of 4-10 ng/ml. This approach will detecet the usual number of cancers with a high TPSA or a suspicious DRE and will lower the cost of diagnosis significantly, a situation, which we believe all urologists should be aware of, especially in this era of economical problems.

Bu çalışmada, prostat kanseri tanısında serbest PSA kullanımının ilave tanısal değerini ve getirdiği maddi yükün belirlenmesini amaçladık İstanbul Üniversitesi Cerrahpaşa Tıp Fakültesi Üroloji Anabilim Dalı'nda total ve serbest PSA (TPSA/ SPSA) ölçümleri olan ve TRUS+biyopsi yapılan 882 olgunun dosyalan retrospektif olarak tarandı. TPSA değerleri 0-3.9 ng/ml, 4-9.9 ng/ml ve >10 ng/ml aralıklarında ve DRE ile şüpheli nodul saptandığı durumlarda SPSA oranı 10 ng/ml ve DRE'de şüpheli nodul bulunan hastalarda kanser oranlan sırasıyla %43.1 ve %32.5 iken, bu gruplarda SPSA oranı <0.15 olan olgularda kanser saptama oranı sırasıyla %25.7 ve %18.1 idi (p=0.01). Bu iki grupta SPSA tetkiki hasta başına tanı maliyetini %100 artırdığı gibi ilave tanısal bir değeri de yoktu. TPSA 4-9.9 ng/ml olan grupta total kanser oranlan ile SPSA oranı < 0.15 olan grupta kanser saptanma oranlan açısından istatistiksel bir farklılık saptanmadı. Ekonomik güçlükler içinde olan ülkemizde her hastadan rutin olarak SPSA tetkiki istenmemesi, bu tetkikin sadece TPSA düzeyi 4-10 ng/ml olan hastalarla sınırlı kalması gerektiği kanısındayız. Bu yaklaşımla, diğer gruplardaki SPSA oranı < 0.15 olan hastalardaki kanser saptanma oranı azalmayacağı gibi, hasta başına tanı maliyetinde de belirgin bir tasarruf sağlanacaktır.
