Residual prostatic tissue ratio after transurethral resection of the prostate in patients with benign prostatic hyperplasia

Abstract
In this study, we aimed to determine how much prostate tissue resection can be achieved at transurethral resection of prostate (TURP) and compared with open prostatectomy by transrectal ultrasonography (TRUS) and the amount of tissue resected. Total 40 male patients with benign prostatic hyperplasia who underwent TURP (n:20) and open prostatectomy (n:20) between January 1999- October 1999 were evaluated prospectively. Four patients were excluded from study because of urethral sitricture and therefore results of 36 patients were evaluated. "International Prostate Symptom Score", digital rectal examination, TRUS, uroflowmetry, blood PSA and residual urine measurement was performed in all patients preoperatively and 3. mount postoperatively. Preoperative and postoperative results were compared. Mean ages of patients was 68.3±2.7 in TURP group and was 68.7±0.9 in open prostatectomy group (p>0.05). Preoperative prostate volume was 39.8±2.8 ml in TURP group and was 69.8+4.0 ml in open prostatectomy group (p<0.0001). The mean weight of prostate tissue removed was 17.1±2.0 g in TURP group and was 49.4±4.3 g in open prostatectomy groups (p<0.0001). The residual prostatic tissue percent was 55.2±4.1 in TURP group and was 28.5±2.9 in open prostatectomy group (p<0.0001). Our results confirms the concept that the of complete resection is not realistic and that only the adenoma in transitional zone is resected at TURP. Although, 26.5% of prostatic adenoma did not resected at TURP compared with open prostatectomy, TURP is efficacious procedure and there is no correlation between the residual prostatic tissue and patients symptoms.

Transüretral prostat rezeksiyonunda (TURP) prostat dokusunun ne kadarının rezeke edilebildiğini transrektal ultrasonografi (TRUS) ve çıkartılan doku miktan tayini yoluyla açık prostatektomi yöntemiyle karşılaştırarak saptamayı amaçladık Ocak 1999 ile Ekim 1999 tarihleri arasında BPH tanısıyla 20'si TURP ve 20'si açık prostatektomi operasyonu uygulanan toplam 40 hasta prospektif olarak değerlendirildi. Her iki gruptan ikişer hasta firetra darlığı gelişmesi nedeniyle çalışma dışı bırakıldı ve 36 hastanın sonuçlan değerlendirildi. Tüm hastalara preoperatif ve postoperatif 3. ayda "International Prostate Symptom Score", parmakla rektal muayene, serum PSA düzeyi, TRUS ile prostat volüm ölçümü, postmiksiyonel rezidüel idrar miktan ve uroflowmetri yapıldı. Preoperatif ve postoperatif bulgular karşılaştırıldı. Hastaların yaş ortalamaları TURP grubunda 68.3±2.7 yıl ve açık prostatektomi grubunda ise 68.7±0.9 yıl idi. Preoperatif prostat volümü TURP grubunda 39.8±2.8 gr ve açık prostatektomi grubunda ise 69.8±4.0 gr idi (p<0.0001). Çıkanlan doku ağırlığı TURP grubunda 17.1±2.0 gr ve açık prostatektomi grubunda ise 49.4±4.3 gr idi (p<0.0001). Kalan doku yüzdesi TURP grubunda 55.2±4.1 ve açık prostatektomi grubunda ise 28.5±2.9 idi (p<0.0001). Sonuçlarımız tam rezeksiyon kavramının gerçekçi olmadığını ve sadece transizyonel zondaki bir adenomun TURP'de çıkanlabildiğini doğrulamaktadır. TURP'de açık prostatektomiye göre adenomun %26.5'unun kalmasına rağmen TURP etkin bir girişimdir ve kalan doku ile hastanın semptomlan arasında doğru orantı yoktur.
