Conservative treatment of invasive carcinoma of the bladder (Clinical stage T2 -T3a) with deep transurethral resection followed by 4 cycles of MVEC chemotherapy

Abstract
In this study we aimed to investigate the efficacy of conservative treatment of invasive carcinoma of the bladder with deep transurethral resection (TUR) and 4 cycles of methotrexate, vinblastine, epirubicin and cisplatin (MVEC) chemotherapy. Conservative treatment with deep TUR followed by 4 cycles of MVEC chemotherapy was considered in 23 patients with clinical stage T2-T3a bladder cancer who refused cystectomy or medically unsuitable for the extensive surgery between May 1990 and January 1999 in our clinic. After 4 weeks 19 patients who were able to complete the protocol were re-evaluated with cytoscopy, TUR of the tumor area and multiple biopsies from other parts of the bladder. Patients were then followed by cytoscopy, intravenous pyelography and computed tomography at regular intervals. Among 19 patients who completed the protocol 15 (79%) were tumor-free at the initial evaluation. Later 7 (36.8%) of these tumor-free cases had tumor recurrences. In the long-term the bladder could be preserved in 12 (63.2%) of the 19 cases for a mean period of 47 (4-93) months. Conservative management with deep TUR and MVEC chemotherapy was found to be beneficial in invasive bladder cancer patients who refused cystectomy.

Bu çalışmamızda kasa invaziv mesane tümörlerinde derin transüretral rezeksiyon (TUR) sonrası 4 kür metotreksat, vinblastin, epirubisin ve sisplatin (MVEC) konservatif tedavisinin etkinliğini araştırmayı amaçladık. Mayıs 1990 ile Ocak 1999 tarihleri arasında evre T2 veya T3a mesane transizyonel hücreli karsinom tanısı konulan ancak sistektomiyi reddeden ya da medikal nedenlerle sistektomiye uygun bulunmayan 23 olguya komplet TUR sonrası 4 kür MVEC kemoterapisi verilmesi planlandı. Bu protokolü tamamlayan 19 olgu 4 hafta sonra sistoskopi, tümör sahasından TUR ve mesaneden çok odaklı derin doku örnekleri alınarak tekrar değerlendirildi. Sonrasında tüm olgular sistoskopi, intravenöz pyelografi ve bilgisayarlı tomografi ile düzenli aralıklarda izleme alındı. Protokolü tamamlayan 19 olgunun 15'inde (%79) tedavi sonrası ilk değerlendirmede mesanede tümör saptanmadı. Bu 15 olgunun 7'sinde (%36.8) ilerleyen dönemlerde mesanede tekrar tümör gelişti. Uzun dönemde 19 hastanın 12'sinde(%63.2) ortalama 47 ay (4-93 ay) süreyle fonksiyonel olarak mesanenin korunması mümkün oldu. Sistektomi yapılamayan kasa ilerlemiş mesane tümörlerinde derin TUR sonrası MVEC tedavisi yararlı bulundu.
