Holmium: Yag laser on treatment of ureteral stones: Our experiences on the 85 patients

Abstract

A total of 85 patients have had holmium: YAG laser treatment for their middle and lower ureteral stones from January 1999 to December 2001. 76 of the stones were located in the lower ureter, while 9 patients had mid-ureteral calculi. All patients were operated on in the standard lithotomy position, under spinal anesthesia. Depending on the physical properties of the stones, a power of 0.5 to 2 Joules and a frequency of 5 to 10 Hertz were applied. In order to measure the effect of the learning curve on the results, cases done before and after May 2000, were evaluated separately. Additionally, the impacts of gender, stone diameter and location on the stone-free status, operation and hospitalization times were investigated. 91.7 percent of the patients were rendered stone free following the first operation. 2 patients had ureteral perforations and one patient suffered a ureteral stricture in the second month postoperatively. The complication rate was 3.5 percent. A double-J stent was introduced in 5 patients for limited mucosal injuries. Patients with mid-ureteral stones and the first 45 patients of the series had a significantly higher hospitalization time, while patients operated on with a safety guide wire in place had a higher stone-free status. Finally, holmium: YAG laser ureterolithotripsy was found to be a safe and effective method. Although it is expensive, its versatility renders the instrument effective for a reference center.

Özet

Ocak 1999 ve Aralık 2001 tarihleri arasında üreter orta ve alt bölüm taşı olan toplam 85 hastaya holmiyum:YAG lazer litotripsi uygulandı. Taşlar 76 olguda alt, 9 olguda ise orta bölüme lokalize idi. Tüm olgularda spinal anesteziyi takiben, standart pozisyon verilerek işlem gerçekleştirildi. Taşın özelliğine bağlı olarak 0.5-2 joule güç ve 5-10 Hz frekansta atım uygulandı. Olgular Mayıs 2000 tarihinden önce ve sonra opere edilenler olarak iki gruba ayrıldı ve deneyimin sonuçlara etkisi araştırıldı. Ayrıca olgular cinsiyet, taş büyüklüğü ve lokalizasyonu ve safety guide wire kullanımına göre gruplandırılarak, taşsızlık; operasyon süresi ve hastanede kalış süresi açısından karşılaştırıldılar. Olguların %91.7'sinde tek seansta taşsızlık sağlandı. Komplikasyon olarak iki olguda üreter perforasyonu ve bir olguda postoperatif 2. ayda üreteral darlık izlendi. Komplikasyon oranı %3.5 olarak bulundu. Olguların 5'ine olası mukozal hasarlanma nedeniyle işlem sonrası double-J stent yerleştirildi. Orta bölüm taşlarının ve opere edilen ilk 45 olgunun hastanede kalış sürelerinin anlamlı olarak fazla olduğu ve safety guide wire kullanılan olgularda taşsızlık oranlarının daha yüksek olduğu saptandı. Sonuç olarak Holmiyum YAĞ ile üreterolitotripsi, güvenli ve etkin olarak bulunmuştur. Pahalı olmasına karşın, çok amaçlı kullanılabilir olması, bir referans merkezi için bu yöntemin fiyat/yarar oranını düşürmektedir.
