The relationship between human leucocyte antigens and renal cell carcinoma

Abstract

Despite of the fact that a lot of risk factors have been suggested on Renal Cell Carcinomas (RCC) their etiologies are still unclear. Especially, RCC in high stages may affect the immune system by causing defects during the formation or presentation of Human Leukocyte Antigens (HLA). HLA may play a complementary role in the activation between the tumor and the immune system. It was our aim to confirm the existence of the relation between the HLA system and RCC in this study. By using the standard microlymphocytotoxicity method of Terasaki in our study, the HLA A, B, DR and DQ antigene types of 20 patients who had RCC in stages T1 and T2, were compared with the control group of 30 healthy people. HLA-A23(9) and DQ7(3) antigens were significantly higher in our RCC patient group when it was compared with the control group statistically.(p=0.005, p=0.028; respectively). HLA-A10, DQ1, DR10 and B44 antigens were significantly higher in the control group than the patient group (p=0.011; for all). The results revealed that the people, carrying the antigens which were detected in the patient group, were highly at the risk of RCC and the antigens which were highly detected in the control group, could be accepted as protective for RCC. A dramatic regression may be observed with the patients who underwent immunotherapy and the effects of immunotherapathic agents may be directed by the HLA expression, which is known to be playing a role in tumor biology. Immunologic description and destruction is avoided in case of change or disappearance of HLA expression by cancer cells. Further investigations, which will be performed in our population in the future will be more illuminating to confirm those results. We have thought that, HLA profiles may be evaluated to detect the people at risk of RCC, the prognosis of the patients and their treatment.

Birçok risk faktörü öne sürülmüş olmasına rağmen Renal Hücreli Karsinomların (RHK) etiyolojileri halen açık değildir. Özellikle yüksek evreli RHK immün sistemi Human Lökosit Antijenlerin (HLA) oluşumu veya sunumunda defektler oluşturarak etkileyebilir. HLA tümör ve vücut savunması arasındaki etkileşimde bütünleyici rol oynayabilir. Biz bu çalışmada HLA sistemi ile RHK arasındaki ilişkinin varlığını göstermeye çalıştık. Çalışmamızda Terasaki'nin standart mikrolenfositotoksisite metodunu kullanarak Evre T1 ve T2 renal hücre karsinomu mevcut olan 20 hastanın HLA A, B, DR ve DQ antijenlerinin tiplerini 30 sağlıklı kontrol grubu ile karşılaştırdık. Renal hücre karsinomlu hasta grubunda HLA-A23 (9) ve HLA-DQ7 (3) antijenleri kontrol grubundan istatistiksel açıdan anlamlı olarak yüksek bulundu (sırasıyla; p=0.005, p=0.028). HLA-A10, DQ1, DR10 ve B44 antijenleri ise kontrol grubunda hasta grubuna göre anlamlı olarak daha yüksek bulundu (hepsinde p=0.011). Bulgularımız renal hücre karsinomlu hasta grubundaki antijenleri taşıyan kişilerin RHK için yüksek risk altında olduğunu, kontrol grubunda yüksek olarak bulunan doku antijenlerinin ise RHK'dan koruyucu özellikte olabileceğini düşündürdü. İmmünoterapi uygulanan hastalarda dramatik bir regresyon olabilir ve tümör biyolojisinde rol oynadığı bilinen HLA ekspresyonu immünoterapötik ajanların etkilerini yönlendirebilir. Kanser hücreleri tarafından HLA ekspresyonunun değişmesi veya kaybolması durumunda immünolojik tanımlama ve yıkım engellenir. Gelecekte popülasyonumuzda yapılacak daha geniş çalışmalar bu bulguların gerçekliği konusunda daha aydınlatıcı olacaktır. HLA profillerinin gelecekte RHK açısından risk altındaki kişilerin saptanmasında ve hastaların prognozu ile tedavileri açısından değerlendirilebileceğini düşünüyoruz.
