The role of temporary filters on tumor thrombosis of inferior vena cava due to renal cell carcinoma

Abstract
Objectives: Renal cell carcinoma, which accounts for 3 % of all adult malignancies, is the most lethal of the urologie cancers. Vena cava thrombosis with renal cell carcinoma occurs in 4% to 10% of patients. We reported our experiences and short time follow up results in patients with tumor inferior vena cava thrombosis secondary to renal cell carcinoma. Before radical nephrectomy, temporary vena cava inferior (VCI) filter was inserted to prevent pulmoner embolism. Materials and methods: We performed temporary VCI filter insertion as an auxiliary procedure to radical nephrectomy, cavatomy and thrombectomy in 2 patients with renal cell carcinoma and extensive tumor thrombosis of the vena cava. 74 and 52 years old patients were referred to our clinic with 8x6x9 cm and 9x10x11 cm, T3b left renal tumors extending into the inferior vena cava. The caval thrombosis were limited and completely below the level of the hepatic veins in both of cases. After preoperative placement of temporary VCI filter at the radiology department, we performed radical nephrectomy, cavatomy and thrombectomy. The VCI filters were carried out after 24 hours from the operation. Results: In both of cases, stage T3bN0M0, Fuhrman grade 3 and 4 renal cell carcinoma were determined in pathologic specimens. 6 units blood transfusion was done for the case 1 and 5 units for the case 2. After the operations ease 1 has been followed for 72 hours and case 2 for 48 hours in the intensive care unit. The mean follow up time was 274 days (188-360). No postoperative early and late complication was noted due to insertion of the filters and to the operations. No patients had any clinical evidence of tumor embolism at the postoperative period. Conclusion: Temporary VCI filter can be inserted easily with a low rate of complication for preventing tumor embolism before radical nephrectomy. This procedure may provide a more safety radical nephrectomy and complete removal of tumor thrombosis from vena cava for the cancer control. Additionally, the risk of pulmoner embolism may be decreased per or postoperatively.

İnferiyor vena kavada tümör trombüsü olan böbrek hücreli karsinomlarda radikal nefrektomi öncesi pulmoner emboli riskini önlemede geçici vena kava İnferiyor (VCI) filtresinin etkinliği değerlendirildi. Yaşlan 74 ve 52 olan, T3b sol böbrek tümörü ve İnferiyor vena kavada tümör trombüsü olan 2 olgu tanıları konmuş olarak kliniğimize refere edildi. Her iki olguda da tümör trombüsü hepatik ven seviyesinin altında bulunmakta idi. Geçici VCI filtresi, ameliyat öncesi radyoloji bölümünde aksiller ven yoluyla perkutan olarak yerleştirildi. Bu işlemden hemen sonra olgular ameliyata alınarak radikal nefrektomi, kavatomi ve trombektomi uygulandı. VCI filtreleri ameliyattan 24 saat sonra perkutan yolla çıkarıldı ve filtrelerdeki trombüsler patolojik incelemeye gönderildi. Patolojik inceleme sonucunda her iki hastada da evre TSbN0M0, Fuhrman grade 3 ve 4 böbrek hücreli karsinom ve vena kavada tümör trombüsü tanısı doğrulandı. Filtrelerden alınan trombus parçalarının tümöre ait olmadığı belirlendi. Ortalama izlem süreleri 274 gün (188-360) olan olgularımız halen hastalıksız olarak takibimiz altındadır. Filtrelerin yerleştirilmesi, çıkarılması ve ameliyata bağlı erken yada geç komplikasyon izlenmedi, Her iki hastada postoperatif dönemde tümör embolisine ait klinik bulgu saptanmadı. Geçici vena kava filtresinin yerleştirilmesi, uygulaması kolay ve komplikasyon oranı düşük bir metoddur. Renal ven ya da subdiafragmatik seviyede vena kava tutulumu olan renal hücreli karsinomlu hastalarda, kanser kontrolü ve pulmoner emboli riski açısından radikal nefrektomi ve trombektominin güvenliğini arttırdığı izlenimini uyandırmaktadır.
