Our treatment results in patients with Wilms' tumor

Abstract

Introduction: Wilms' tumor is the most common pediatric urologic tumor representing 8 percent of pediatric solid tumors. 80% of patients identified before 5 years of age. Bilateral synchronous Wilms' tumor occurs in 5% of patients with Wilms' tumor. WT1 (11p13) and WT2 (11pl5) genes are felt to be tumor suppressor genes and their loss is implicated in the development of Wilms' tumor. Materials and Methods: Between April 1995 to February 2003 17 patients diagnosed as Wilms' tumor and examined for clinicopathological characteristics and followed for the treatment results in Dr. Lütfi Kırdar Kartal Research and Education Hospital. There were 7 male and 10 female patients ages ranged 1-11 years with a mean of 4.7 year. We performed transabdominal-transperitoneal approach to all patients. We examined controlateral kidney, liver and other abdominal organs for metastases with inspection and palpation. We explored renal vein and vena cava inferior for tumor thrombus. Limited lymph node sampling was performed for suspicious cases. All cases underwent nephrectomy and postoperative appropriate chemotherapy and/or radiotherapy performed according to their stage and histological tumor type. 3 cases received chemotherapy preoperatively. in one patient tümör rupture occurred during surgical dissection in the operation. We detected tümör thrombus in vena cava inferior and it was removed completely in one patient. Results: Two patients were stage I (11.7%), 6 patients were stage II (35.2%), 4 patients were stage III (23.5%), 3 patients were stage IV (17.6%), 2 patients were stage V (11.7%). 15 patients had favorable histological type and 2 patients with unfavorable histopathological type. 3 patients had distant metastatic lesions, l with hepatic and 2 with pulmonary metastasis. We lost 2 patients (11.7 %), the causes of dead was hepatotoxic reasons during chemotherapy in one patient and gastrointestinal system infection in other patient. 3 patients (17.6 %) were lost on follow-up. 8 patients (47.0%) finished treatment with full remission and 4 patients (23.5%) on full remission with unfinished treatment. Overall 5-year survival rate for Wilms' tumor was 84% and it can be clarified in stages as 100% for stage I, 100% for stage II, 75% for stage III, 66% for stage IV, 100% for stage V. Conclusion: Recently there has been importaht improvement in the treatment and survival of Wilms' tumor. This is likely due to combination of many factors including standardization of treatment, appropriate chemotherapy and radiotherapy according to the tumor stage and histological tumor type, surgical experience and improved postoperative care.

Wilms tümörü en sık görülen pediyatrik ürolojik tümördür ve tüm pediyatrik solid tümörlerin %8'ini oluşturur. Hastaların %80'i 5 yaşından önce teşhis edilir. Bilateral eşzamanlıWilms tümörü, hastaların %5'inde görülür. WT1 (11pl3) ve WT2 (11p15) genlerinin tümör supresör genleri olduğu düşünülmektedir ve Wilms tümör gelişiminde bu genlerin yokluğu suçlanmaktadır. Nisan 1995 ile Şubat 2003 tarihleri arasında Dr. Lütfi Kırdar Kartal Eğitim ve Araştırma Hastanesi Üroloji Kliniği'nde Wilms tümörü tanısıyla tedavi edilen, yaşları 1 ile 11 yaş (ortalama 4,7) arasında değişen 7'si erkek, 10'u kız olmak üzere toplam 17 hasta klinik özellikleri ve tedavi sonuçlan açısından incelendi. Bütün hastalara transabdominal-transperitoneal yaklaşım uygulandı. Karşı böbrek, karaciğer ve diğer abdominal organlar metastaz varlığı açısından inspeksiyon ve palpasyon ile kontrol edildi. Renal ven ve inferiyor vena kava tümör trombusu açısından değerlendirildi. Şüpheli vakalarda sınırlı lenf nodu diseksiyonu yapıldı. Bütün hastalara nefrektomi yapıldı ve klinik evre ve histolojik tümör tipine göre ameliyat sonrası uygun kemoterapi ve/veya radyoterapi uygulandı. 3 hastaya ameliyat öncesi kemoterapi verildi. Bir hastada operasyon sırasında diseksiyon yapılırken tümör rüptürü gerçekleşti. Bir hastada inferiyor vena kavada tümör trombusu tespit edildi ve tamamen çıkarıldı.Hastalardan 2'si evre I (%11,7), 6'sı evre II (%35,2), 4'ü evre III (%23.5), 3'ü evre IV (%17.6), 2'si ise evre V (%11.7) idi. 15 hasta "favorable", 2 hasta "unfavorable" histopatolojik özellik gösteriyordu. Metastatik hastalığı olanların l tanesinde karaciğer, 2 tanesinde akciğer tutulumu vardı. Bir hasta kemoterapi esnasında gelişen hepatik toksisite, diğer bir tanesi de gastrointestinal sistem enfeksiyonu nedeniyle olmak üzere toplam 2 hasta (%11.7) kaybedildi. 3 hasta (%17.6) takipten çıkartıldı. Halen 8 hasta (%47.0) tam remisyonda tedavisi bitmiş olarak izlenmekte ve tam remisyon kabul edilen 4 hastanın (%23.5) ise tedavisi devam etmektedir. Hastaların 5 yıllık sağkalım oranı genel olarak bakıldığında %84, evrelere göre ise evre I'de %100, evre H'de %100, evre IU'de %75, evre IV'de %66 ve evre V'de %100 olarak bulunmuştur. Tedavideki standardizasyon, evre ve histolojiye uygun şekilde tedavi, cerrahi deneyim ve ameliyat sonrası bakımdaki gelişmeler gibi bir çok faktöre bağlı olarak son yıllarda Wilms tümörü tedavisinde önemli gelişmeler olmuş ve sağkalım oranı artmıştır.
