Toxicity and result of MVAC chemotherapy in advanced transitional cell urethelial carcinoma

Abstract

Introduction: MVAC (methotrexate, vinblastin, adriamycin and cisplatin) is a standard treatment in advanced transitional cell carcinoma of the urethelium. The experience with MVAC chemotherapy in advanced transitional cell urothelial carcinoma is reviewed with emphasis on toxicity and effıcacy. Materials and Methods: Between January 1998 and April 2003, 39 patients (37 men, 2 women) who had histologically proven, progressive disseminated TCC of bladder (37) and ureter (2) were treated with MVAC. The chemotherapy scheme was: Methotrexat 30 mg/m2, days l, 15, and 22; vinblastine 3 mg/m2, days 2, 15, and 22; doxorubicine (adriamycine) 30 mg/m2, day 2; cisplatin 70 mg/m2, day 2. Median age value was 65 years (range betvvcen 50 and 76). Patients had a bidimensional measurable lesion of at least 2.5 cm. Patients with only bone lesions were not included. Prior systemic chemotherapy or immunotherapy w as not allovved. Dose modifications depended on toxicity according to WHO (World Health Organization) criteria. Respond evaluations (WHO criteria) were performed after every 2 cycles, and further therapy depended on the response. The maximum number of cycles was 6. Median follow-up period was 24.1 months (range 0-58). We reported on toxicity and result during the MVAC treatment. Results: The average number of cycles was 4.4. Complete response was seen in 8 patients (20.5%), partial response in 7 (17.9%), stable disease in 10(25.6%), progression in 10(25.6%) and 4 patients were not evaluated for response. Relapses were frequent (9 of 14 remaining responders, 64.3%). Leucopenia was the most frequent and severe side effect (Gl leucopenia %24, G2 %32, G3 %15, G4 %9). One patient (progressive) died of urosepsis during the third course, a second patient (partial responder) died of urosepsis during fourth course. Thrombocytopenia grade l was seen in 12% (7-14%), grade 2 in 14%(4-18%), grade 3 in 5%(2-7%), grade 4 in 3%(0-7%). in 4 patients MVAC therapy was stopped because of a nephrotoxicity. 3 patients left the treatment due to malasia. in total, 18 patients (46%) were hospitalized due to toxicity, and there were 2 toxic deaths. Other side effects were acceptable and could be treated successfully. Median survival of the whole group was 14(0-58) months, without a significant difference for responders and nonresponders (p=0.07). Conclusion: Response rate of MVAC chemotherapy between 40 and 70 % was reported in the literatüre. The patient selection is different in the numerous clinical studies, so it is difficult to compare the result of these studies. Response rate was 38.4% in our study. Although MVAC chemotherapy remains a toxic and potentially lethal therapy, it is effective with tolerable side effects.

MVAC (Metotreksat, Vinblastin, Adriamisin, Sisplatin) ileri evre değişici epitel karsinomlannın standart tedavisidir. İleri evre değişici epitel karsinomlarmda MVAC toksisite ve etkinliğini değerlendirmeyi amaçladık. 1998 Ocak- 2001 Nisan tarihleri arasında, mesane (37) ve ureter (2) ileri evre değişici epitel karsinomu olan 39 hastaya MVAC kemoterapisi verildi. Metotreksat l, 15 ve 22. günlerde 30 mg/m2; vinblastin 2, 15 ve 22. günlerde 3 mg/m2; doksorubisin (adriamisin) 2. günde 30 mg/m2; sisplatin 70 mg/m2 2. günde verildi. Hastaların ortalama yaşı 65 (50-76) idi. Hastalarda iki boyutta ölçülebilen en az 2,5 cm uzunluğunda lezyon vardı. Sadece kemik tutulumu olan, immunoterapi alan, kemoterapi alan hastalar çalışmaya alınmadı. WHO kriterine uygun toksisiteye göre doz modifikasyonu-yapıldı. Her iki kürden sonra WHO kriterlerine göre yanıt değerlendirildi. Yanıta göre tedavi planlandı. En fazla 6 kür verildi. Hastalar ortalama 24.1 (0-58) ay izlendi. Tedavi sırasında izlenen toksisite ve tedavi sonuçlan bildirildi. Ortalama kür sayısı 4.4 idi. Tam yanıt 8(%20.5) hastada, kısmi yanıt 7(%17.9) hastada, yanıtsızlık 10(%25.6) hasta, ilerleme 10(%25.7) hastada görüldü, 4(%10.2) hasta değerlendirilemedi. Nüks sıklıkla izlendi (14 yanıt veren hastanın 9'unda, %64.3). Lökopeni en-sık ve en ciddi yan etki olarak görüldü (Gl lökopeni %24,G2 %32, G3 %15,04 %9). İlerleme gösteren bir hasta ürosepsis nedeniyle üçüncü kürde, kısmi yanıtlı bir hasta dördüncü kürde ürosepsis nedeniyle kaybedildi. %12 hastada grade l (%7-14), %14 hastada grade 2 (%4-18), %5 hastada grade 3 (%2-7), %3 hastada grade 4 (%0-7) trombositopeni görüldü. Nefrotoksisite nedeniyle 4 hastada tedavi durduruldu. Genel halsizlik nedeniyle 3 hasta tedaviyi sürdüremedi. Toplamda 18 (%46) hasta toksisite nedeniyle hospitalize edildi. Diğer yan etkiler kabul edilebilir ve başarılı bir şekilde tedavi edilebilen yan etkilerdi. Tüm gruplarda median yaşam süresi 14 (0-58) ay idi, yanıt veren ve yanıt vermeyen gruplar arasında anlamlı fark bulunmamıştır (p=0.07). MVAC ile %40-70 yanıt oranları bildirilmektedir. Çalışmalarda hasta seçimindeki farklılıklardan dolayı elde edilen verileri kıyaslamakta güçlükler yaşanmaktadır. Çalışmamızda %38.4 yanıt oranı başarılmıştır. Toksisite MVAC kemoterapisinde sık izlenmekte ve en önemli toksisisite lökopeni kabul edilmektedir. MVAC tedavisi toksik ve ölümcül olabilmesine rağmen, tolere edilebilen yan etkileri ile etkili bir tedavidir
