The importance of training before clinical experience in laparoscopic surgery: Heilbronn laparoscopic training program

Abstract

Introduction: In 1990's, after the first description of laparoscopic nephrectomy and especially proved the effectiveness of laparoscopic radical prostatectomy, an increasing number of laparoscopic procedures were performed worldwide. Nowadays, most of the urological centers are asked to offer the laparoscopic option in order to fulfill patient requests. Nevertheless, there still exists a significant lack of standardized training programs to teach ablative and most important reconstructive laparoscopic operations. The lack of standardized training programs in laparoscopy created a situation in which every hospital or laparoscopic center develop their own training programs not always exposing the student to various necessary skills. This situation led us to develop an animate training program composed of six standardized stages. We evaluate and validate a new standardized step-by-step program, "Heilbronn Laparoscopic Training Program", to improve laparoscopic skills and enable trainees not experienced in laparoscopy to perform finally an urethrovesical anastomosis. Materials and Method. In an inanimate model, 5 participants were exposed to six different basic and reconstructive «xercises. The different steps consist of improvement of hand-eye coordination, linear and curved suturing with changing angles of the needle. The final step enabled the participants to perform an accurate urethrovesical anastomosis in a porcine bladder model. During 2003, the expert laparoscopic (first and second generation) surgeons in our hospital developed the various stages of the training program in order to define the expert level time for each stage. The time frames 3,15,15,10, 20 and 30 minutes for steps I, II, III, IV, V and VI, respectively. Results: After a mean time 23 hours of training, all participants were able to perform all steps in time and completed an accurate urethrovesical anastomosis less than 30 minutes. The time required to success the program before and after training revealed a significant decrease (p<0.05). Continual training program of reconstructive procedures decreased the time for suturing activities 70%. The time for the intracorporeal knotting activities could be decreased 40%. Conclusion: Dealing with laparoscopy the common concept of "see one, do one, teach one" is not applicable in practice. To fill this gap, we designed teaching concepts based on our experience gained with experimental and clinical researchers made in our clinic which offers a standardized non-clinical and clinical education. "Heilbronn Laparoscopic Training Program" enabled participants not experienced in laparoscopy to increase reproducible performance in reconstructive laparoscopy. These results indicate that especially the challenging parts of reconstructive laparoscopy such as intracorporeal suturing could be taught using standardized concept. This experience could be incorporated easily in every department developing a laparoscopic training program. Additionally, our step-wise training program concentrating on the non-clinical phase has to followed by a clinical phase.

Özet

1990'lı yıllarda laparoskopik nefrektominin tanımlanması ve özellikle laparoskopik radika' prostatektominin geçerliliğinin de ortaya konulması ile ürolojideki laparoskopik girişimlerin sayış giderek artmıştır. Bu' hızlı ilerlemeye paralel olarak ne yazık ki standardize edilmiş bir eğitin programındaki gelişim yetersiz kalmıştır. Bu çalışmada daha önceden laparoskopik eğitimi almamı: adayların laparoskopik tecrübelerini geliştirmek üzere standardize edilmiş ,ve son aşamasındi uretrovezikal anastomuz (UVA)adımıyla sonuçlanan "Heilbronn Laparoskopik Eğitim Programı"nn etkinliği araştırılmıştır.Cansız eğitim programında, 5 aday altı aşamalı temel ve rekonstrüktif eğitime tabii tutulmuşlardır. Her bir aşama el-göz koordinasyonu, farklı açılarla sutür atma teknikleri ve son aşamada domuz modelinde UVA gibi farklı tecrübe edinimleri üzerine oluşturulmuştur. Bir sonraki aşamaya geçmek için, altı aşamada sırasıyla 3, 15, 15, 10, 20 ve 30 dakikalık süre diliminde hedeflenen eğitimin bitirilmesi amaçlanmıştır. Ortalama toplam 23 saatlik eğitim sonrasında, tüm adaylar 30 dakikada istenilen niteliklere uygun :UVA gerçekleştirmişlerdir. Eğitimin tamamlanması ile, adayların önceki ve sonrasındaki elde ettikleri hız anlamlı derecede farklılık göstermiştir. Devamlı ve kademeli uygulanan rekonstrüktif eğitim yaklaşımı, UVA için harcanan zamanın yaklaşık %70 azaltmakla birlikte, düğüm aşamasında bu %40'lık bir hızlanma ile kendisini göstermektedir. Tarif edilen "Heilbronn Laparoskopik Eğitim Programı" ile daha öncesinde ciddi bir tecrübeye sahip olmayan katılımcıların rekonstrüktif laparoskopik performanslarında önemli gelişme ve hızlanma sağlanmaktadır. Elde edilen bu sonuçlarla özellikle laparoskopik cerrahideki rekonstrüktif aşamada intrakorporeal sutür tekniğinin eğitiminde standardizasyon kazanılmasında önemli aşama sağlayacaktır. Bunun yanında, tarif, edilen program laparoskopik eğitim programı oluşturmak isteyen her klinikte kolaylıkla uygulanabilecek bir programdır.
