PSA and PSMA levels of circulating cells assesed by reverse transcriptase-polymerase chain reaction (RT-PCR) in prostate cancer

Abstract

Introduction: Currently reverse transcription polymerase chain reaction (RT-PCR) for prostate specific antigen (PSA) and prostate specific membrane antigen (PSMA) messenger RNA are being evaluated for detecting circulating prostate cells. The aim of the study is to detect PSA and PSMA levels in patients with prostate cancer by nested RT-PCR and evaluate their association with clinic patient characteristics. Materials and Methods: A total of 36 men diagnosed to have prostate cancer were enrolled to the study before initiation of any treatment and assessed by digital rectal examination, serum PSA (immunoassay), fPSA/PSA ratio, transrectal ultrasonogrophy guided biopsy and by bone scan and abdominal CT when indicated. During transrectal ultrasonography, biopsy was made in 21 patients with pathological findings in rectal examination and 15 patients with high serum PSA levels. Control group consisted of 10 women and 5 men with BPH. A 5-10 ml. peripheral blood specimen was obtained from all patients. PSA and PSMA mRNA levels of circulating cells were assessed by nested RT-PCR in all cases and results were compared with serum PSA, clinical stage, Gleason score and presence of lymph node or distant organ metastasis. Results: The mean age and serum PSA levels of men with prostate cancer were 69.3±4.5 (62-80) years and 101.5± 406.6 (3.79-2464) ng/ml, median 24.87 ng/ml, respectively. Clinical staging revealed on T2 disease in 10 (27.7%), T3 disease in 10 (27.7%) and T4 disease in 16 (44.4%) cases. Radiological evaluation documented lymph node involvement in 20 (55.5%) and distant organ metastasis in (8.3%) cases. RT-PCR PSA levels were positivite in 15 (41.6%) and PSMA levels were positivite in 21 (58.3%) patients. RT-PCR positivite PSA and PSMA by RT-PCR resulted positive in a higher percent of cases with distant organ metastasis (p<0.05). Positive PSA and PSMA levels by RT-PCR were observed in a higher percent of patients in clinical stage 4 when compared to stage 2 and 3 disease. Patients with lymph node involvement exhibited higher PSA and PSMA RT-PCR (p<0.05). RT-PCR results did not reveal any significant correlation with serum PSA, fPSA/tPSA and Gleason score. In control group RT-PCR revealed negative results in all. Of 15 patients with positive RT-PCR PSA levels, 13 had (86.6%) lymph node metastasis, 12 had (80%) distant metastasis. Of 21 patients with positive RT-PCR PSMA levels, 15 (71.4%) had lymph node metastasis and 14 (66.6%) had distant metastasis. Conclusion: PSA and PSMA can be detected in a significant percent of prostate cancer patient with lymph node involvement and distant organ metastasis by RT-PCR. Our preliminary results indicate this method can be efficiently used in the diagnosis of advanced disease and their follow up. Further research is necessary using a much larger number of patients with a longer follow up.

Özet

Günümüzde ‘’Reverse Transcriptase Polymerase Chain Reaction‘‘ (RT-PCR) yöntemi ile dolaşımdaki prostat kanser hücrelerindeki Prostat Spesifik Antijen ve Prostat Spesifik Membran Antijeni (PSMA)’nin mRNA’ları saptanabilmektedir. Bu çalışmada prostat kanserli hastalarda nested RT-PCR ile bakılan PSA ve PSMA düzeylerinin evrelendirme ve olguların özelliklerine göre değişimi incelendi. Kliniğimizde prostat kanseri tanısı alan 36 olgu tedaviye başlamadan önce parmakla rektal muayene, serum PSA (immunoassay), sPSA/tPSA oranı, transrektal ultrasonografi ve biyopsi, gereğinde kemik sintigrafisi ve abdominal BT ile değerlendirildi. Hastaların 21’ne rektal muayenede patolojik bulgu, 15’e ise serum PSA yüksekliği nedeniyle transrektal ultrasonografi (TRUSG) eşliğinde biyopsi yapıldı. Kontrol grubu olarak 15 olgu (10 kadın, 5 BPH hastası) çalışma kapsamına alındı. Bu yöntem için tüm hastalardan 5-10 cc venöz kan alındı. Tüm olgularda Nested RT-PCR yöntemiyle dolaşımdaki hücrelerdeki PSA ve PSMA mRNA düzeyleri incelendi. Elde edilen sonuçlar hastaların PSA, klinik evre, Gleason skoru, lenf nodu tutulumu ve metastaz durumuna göre değerlendirildi. Çalışmaya alınan prostat kanserli olguların ortalama yaşı 69.3±4,5 (62-80) yıl, ortalama PSA 101.5±406.6 (3.79-2464) ng/ml, medyan 24.87 ng/ml idi. Klinik olarak olguların 10’u (%27.7) T2, 10’u (%27.7) T3, 16’sı (%44.4) T4 evresindeydi. Radyolojik olarak 20 (%55.5) olgu da lenf nodu 3 (%8.3) olguda uzak metastaz tespit edildi. Çalışmaya alınan olguların serumları RT-PCR ile incelendiğinde toplam 15 (%41.6) hastada RT-PCR PSA antijeninin pozitif olduğu, 21 (%58.3) hastada RT-PCR PSMA antijeninin pozitif olduğu saptandı. Evre 4’deki RTPCR PSA ve PSMA pozitifliği evre 2 ve 3 göre anlamlı olarak yüksek olduğu ve bu pozitifliğin evre arttıkça yükseldiği tespit edildi. Lenf nodu pozitif ve uzak metastazı olan hastalarda RT-PCR PSA ve PSMA düzeyleri anlamlı olarak pozitif bulundu (p<0.05). RT-PCR sonuçlarıyla, serum PSA, sPSA/tPSA ve Gleason skoru arasında istatistiksel ilişki saptanmadı. Kontrol grubu olarak alınan 15 olgunun hepsinde hem RT-PCR PSA hem de RT-PCR PSMA düzeyleri negatif bulundu. RT-PCR PSA pozitif olan 15 hastanın 13’ünde (%86.6) lenf nodu metastazı olduğu, 12 (%80) hastada ise uzak metastaz olduğu saptandı. Benzer şekilde RT-PCR PSMA pozitif olan 21 hastanın 15’de (%71.4) lenf nodu metastazı olduğu, 14 (%66.6) hastada ise uzak metastaz olduğu gözlendi. RT-PCR yöntemiyle ileri evredeki lenf nodu ve uzak metastazı olan hastalarda yüksek oranda dolaşımdaki hücrelerde PSA ve PSMA mRNA’sı tespit edilebilmektedir. Bulgularımız özellikle ileri hastaların tanısında ve tedavilerinin takibinde bu yöntemin etkili olabileceğini düşündürmektedir ancak bu konuda uzun dönem takipli çok sayıda çalışmaya ihtiyaç vardır.
