The effect of sildenafil citrate on brain perfusion

Abstract
Introduction: Sildenafil citrate (Sc), a selective inhibitor of cyclic guanosine monophosphate (cGMP)-hydrolyzing phosphodiesterase 5 (PDE5), is the first oral medication proven effective in the treatment of erectile dysfunction (ED). Data from resources support the proposal that enhancement of penile erection by Sc in patients with ED involves potentialisation of the nitric oxid (NO) stimulated cGMP during sexual stimulation. However, whether it has any effect on the brain function is not clear. In this study the effect of Sc on the brain perfusion was evaluated. Sixty-four year old man with headache localized on the frontal area was referred for regional cerebral blood flow (rCBF) 99m-technetium-hexamethylpropyleneamine oxime (Tc99m-HMPAO) single photon emission computerized tomography (SPECT). He had erectile dysfunction secondary to prostatectomy and had been using Sc for last 2 years. Brain MRI findings were normal. Brain SPECT studies at two different occasions were studied. At the first step, a baseline (Sc-) Tc99m-HMPAO SPECT study was performed. After one week, brain perfusion SPECT was repeated 1 hour after oral administration of 50 mg Sc (Sc+). SPECT images were visually analyzed. Minimal perfusion defect was observed on left frontoparietal cortical area. In Sc+ study, prominent increased Tc99m-HMPAO uptake in right nucleus lentiformis and decreased activity uptake on the right thalamus relative to left and Sc- study was observed. Perfusion defect on the left frontoparietal cortical area in Sc+ study was much more prominent than the defect observed on Sc- study. In rCBF SPECT study performed after the drug administration, a quite prominent radioactivity accumulation was observed on the nasal area which was an indication of dilatation in the extracerebral arteries. In conclusion, alteration on the brain perfusion at the time of maximal effect of drug suggests that Sc has an effect on brain perfusion in addition to its effect of increasing the perfusion locally in the corpus cavernosa. Visualisation of nasal congestion supports this idea too. Similar studies will be valuable for physio-anatomical mapping of brain related with sexual functions.

Sildenafil sitrat (SS), siklik-guanozin monofosfatı (sGMP) hidrolize eden posfodiesteraz 5 (PDE5) enziminin selektif inhibitörüdür. Erektil disfonksiyon (ED) tedavisinde etkinliği kanıtlanmış ilk oral preparattır. Araştırma sonuçları, ED’li hastalarda SS’nin oluşturduğu penil ereksiyondaki artışın, seksüel stimülasyon sırasında oluşan sGMP’nin stimüle ettiği nitrik oksidin potansiyalize edilmesine bağlı olduğu önerisini desteklemektedir. Fakat beyin perfüzyonu üzerine etkisinin olup olmadığı çok açık değildir. Bu çalışmada SS’nin beyin perfüzyonuna etkisi değerlendirilmiştir. Frontal bölgede lokalize baş ağrısı şikayeti olan 64 yaşındaki erkek hastaya 99m-teknesyum hekzametilpropilaminoksim (Tc99m HMPAO) tek foton emisyon bilsisarlı tomografi (SPECT) ile bölgesel beyin kan akımı (rCBF) çalışması yapılması istendi. Hastanın beyin MRI çalışması normaldi. İki farklı durumda beyin perfüzyon çalışması yapıldı. Önce, birinci basamakta SS verilmeksizin beyin perfüzyon sintigrafisi çalışıldı (SS-). Bir hafta sonra 50mg oral SS verildikten 1 saat sonra beyin SPECT çalışması tekrar edildi (SS+). Elde edilen transaksiyal kesitler görsel olarak değerlendirildi. SS- çalışmada sol frontoparyetal kortikal alanda perfüzyon defekti izlendi. SS+ çalışmada sağ nükleus lentiformiste Tc99m HMPAO tutulumunda artış, sağ talamusta ise simetriğine SS- çalışmaya oranla belirgin perfüzyon azalması izlendi. SS+ çalışmada sol frontoparyetal kortikal bölgedeki perfüzyon azalması SS- çalışmadakine oranla daha belirgin idi. SS+ rCBF SPECT çalışmasında nazal bölgede, ekstaserebral arter dilatasyonunun belirtisi olan, belirgin radyoaktivite akümülasyonu izlenmiştir. Sonuç olarak, SS’nin maksimum etkisinin görüldüğü 1. saatteki rCBF değişiklikleri, SS’nin korpus kavernozada lokal perfüzyon artışı etkisine ek olarak beyin perfüzyonu üzerinde de etkisinin olduğunu göstermektedir. Nazal konjesyonun görüntülenmesi bu fikri desteklemektedir. Benzer çalışmalar, beynin seksüel fonksiyonlar ile ilgili fizyo-anatomik haritalamasının yapılabilmesi için değerli olacaktır.
