Urodynamic findings in children with cerebral palsy

Abstract

Introduction: Cerebral palsy is a non-progressive motor disorder caused by a central nervous system insult sustained during gestation or early childhood period. it is characterized by a variety of motor abnormalities. it has been known that more than a third of the children with cerebral palsy are expected to present with dysfunctional voiding symptoms. Recent data suggests that both clinical evaluation and the urodynamic study of cerebral palsy patients should be performed in order to define the type of neurological deficit and to document its effect on voiding function. We have designed this study to assess the symptoms of voiding dysfunction of our cerebral palsy patients. Materials and Methods: in this study, in our department 36 children with cerebral palsy were evaluated with urodynamic studies consisting of the flow rate, filling and voiding cystonietry and electromyographic findings of external urethral sphincter to determine lower urinary tract functions by the same physician. After urodynamic studies, all of the methods and terminations were defined based on International Continence Society (ICS). Treatment of the patients was done according to the urodynamic fındings. in order to decrease and/or to prevent involuntary detrusor contractions and also to increase functional capacity of the bladder, anti-cholinergic medications were used as a treatment of choice. The treatment modalities as clean intermittent catheterization (CIC) and behavioral modification therapy were used in the cases of incomplete emptying. The patients with documented urinary tract infections were treated with efficient antibacterial agents. Prophylactic medication was used in some patients. Results: Of the study group, 16 were giriş and 20 were boys. The mean age of the patients at referral was 8.2 years. Of children underwent this study, 24 (66.6%) had been found to have dysfunctional voiding symptoms. Daytime urinary incontinence (47.2%) and difficulty urinating (44.4%) were the commonest presenting symptoms, respectively. Urodynamic fındings of the children showed that neurogenic detrusor overactivity (involuntary contractions during bladder filling) with reduced bladder capacity was encountered in 17 (47.2%) children, while detrusor-sphincter dyssynergia (DSD) was seen in 4 patients (11%). Based on urodynamics, the mean bladder capacity of these patients was found to be 52.2% of the expected cystometric capacity. in 19 patients (52.7%) cystometric capacity was in normal ör higher level. Conclusion: Our present study showed that voiding dysfunction was found to be in more than half of the children with cerebral palsy in parallel to the literature. We propose that providing a rational and an effective plan of management of these patients depends on the evaluation of the lower urinary tract dysfunction with urodynamic studies. These children benefit from earlier referral for assessment and treatment.

Serebral palsi, gebelik sırasında veya erken çocukluk döneminde meydana gelen merkezi sinir sistemi hasarı ile oluşan ve progresif olmayan bir hastalıktır. Serebral palsili çocukların üçte birinden fazlası disfonksiyonel işeme semptomları ile başvururlar. Bu hastalarda nörolojik hasarların türünü tanımlamak ve bunun işeme fonksiyonu üzerindeki etkisini belirlemek için iyi bir klinik değerlendirmenin yanında yeterli ölçüde ürodinamik incelemenin de yapılması gerekmektedir. Çalışmamızda serebral palsili çocuk hasta grubunda izlenebilecek olan işeme bozukluğu semptomlarının değerlendirilmesi amaçlandı. Özel bir spastik çocuklar merkezinde bakım ve eğitim görmekte olan serebral palsili 36 çocuk hastaya kliniğimizde alt üriner sistem fonksiyonlarını değerlendirmek amacı ile tek bir araştırmacı tarafından işeme hızı, dolum ve işeme sistometrisi ve sfınkter aktivitesi hakkında bilgi edinmek için yüzeyel elektromyografiyi içeren tüm ürodinamik inceleme uygulandı. 16'sı kız, 20'si erkek olan çocukların ortalama yaşı 8.2 yıl idi. Ürodinamik çalışmayı takiben, tedavi protokolleri hastanın gerek öykü, gerekse fizik muayene bulgularına ve ürodinamik çalışmada saptanan bulgulara göre belirlendi. İnhibe edilemeyen istem dışı kontraksiyonları baskılamak veya azaltmak ve fonksiyonel mesane kapasitesini artırmak için antikolinerjik ilaç tedavisine gerekli durumlarda temiz aralıklı kateterizasyon (TAK) ve işeme davranış tedavisi de eklendi. Üriner sistem infeksiyonu olan çocuklara ise uygun antibiyotik tedavisi uygulandı. Bazı çocuklarda ise profilaktik tedavi de verildi.Hastaların 24'ünde (%66.6) işeme bozukluğu vardı. En sık görülen bulgular sırası ile idrar kaçırma (%47.2) ve idrar yapmakta zorluk (%44.4) olarak bulundu. Ürodinamik inceleme sonucunda 17 hastada (%47.2) düşük kapasiteli aşırı aktif mesane -nörojenik detrusor overaktivitesi- (mesane dolumu sırasında oluşan istemsiz detrusor kontraksiyonu) saptanırken, 4 hastada (%11) detrusor-sfınkter dissinerjisi (DSD) bulundu. Hastaların ortalama mesane kapasitesi ise olması gereken ortalama mesane kapasitesinin hemen hemen yarısı kadar (%52.2) olarak hesaplandı. 19 hastada (%52.7) ise sistometrik kapasite normal yada normalden yüksek bulundu.Çalışmamızda serebral palsili çocukların yaklaşık olarak üçte ikisinde işeme bozukluğu bulguları saptandı. Bu hastaların rasyonel ve etkili tedavisi işeme bozukluğunun Ürodinamik inceleme yardımı ile tanımlanmasına bağlı olacaktır. Tedaviye erken başlanılması da gerek hayat kalitelerinin yükselmesine, gerekse hastalığın getirdiği olası komplikasyonlardan uzak durmalarına yardımcı olacaktır.
