Efficacy and safety of caudal anaesthesia performed with 0.1% bupivacain in transrectal ultrasography-guided prostate biopsy

Abstract
Introduction: A lot of techniques have been suggested for pain control during multiple transrectal prostate biopsies. In the present pilot study, we aimed to evaluate the efficacy and safety of caudal anaesthesia with combination low concentration of local anesthetic and opioid for transrectal ultrasound guided prostate biopsy. Materials and Methods: In the Urology Clinic of Atatürk University Hospital, 50 patients underwent transrectal ultrasound guided prostate biopsy because of increased prostate specific antigen or abnormal finding at the rectal examination. 15 ml 0.1% bupivacaine with 50 µg fentanil was injected into caudal space of the all patients 20 minutes before the procedure. Their pain scores were assessed using a visual analog pain scale during probe insertion into anal canal, probe maneuver and biopsies separately. The degree of motor block was assessed with the Bromage scale Results: Visual analog pain scale scores were assessed during the all stages of the procedure as 0-2. Incidences of the scores of 1, 0 and 2 during the probe insertion into anal canal were 24 (48%), 24 (48%) and 2 (4%), respectively. The scores of 1, 0 and 2 during the probe maneuvers were 34(68%), 14(28%) and 2 (4%), respectively. The scores of 1, 0 and 2 during the biopsies were 30 (60%), 14 (28%) and 2(4%), respectively. The patients' satisfactions were bad 1 (2%), good 12 (24%) and very good 37(74%). Bromage scale was 0 for all patients. Three patients experienced urinary retention. Conclusion: There is no doubt that it is of great importance that a proper method of anesthesia which will improve patient tolerance of this procedure, be quick in onset of action, have an easily reversible effect and result in minimum side effects, while allowing it to be performed on an outpatient basis be developed for the daily practice. Caudal anesthesia is used to provide perioperative analgesia for procedures below the umbilicus. Caudal anesthesia with combination low concentration of local anesthetic and opioid is effective and reliable method for preventing the pain during transrectal ultrasonography-guided prostate biopsy.

Transrektal prostat biyopsisi işleminde ağrının kontrolü için birçok yöntem önerilmektedir. Bu pilot çalışmada, düşük doz lokal anestezik ve opioid kombinasyonu ile yapılan kaudal anestezinin transrektal prostat biyopsisi sırasında kullanımının etkinliğini ve güvenirliği araştırıldı. Atatürk Üniversitesi Hastanesi Üroloji Kliniğinde prostat spesifik antijen yüksekliği veya rektal incelemede özelliği olması nedeniyle 50 hastaya transrektal ultrosonografi eşliğinde 12-16 kadran prostat iğne biyopsisi yapıldı. Bütün hastalara işlemden 20 dakika önce 15 mL 50 µgr fentanil içeren %0.1'lik bupivakain solüsyonu kaudal olarak enjekte edildi. Hastaların ağrı skorları vizüel analog skala (VAS) prob girişi, probun hareketi ve biyopsi sırasındaki iğne girişi için ayrı ayrı değerlendirildi. Motor blok ise Bromage skalası ile değerlendirildi. VAS skoru işlemin tüm aşamalarında 0-2 arasında değerlendirildi. 0, 1 ve 2'lik VAS skorlarının dağılımı, prob girişi sırasındaki sırasıyla 24 (%48), 24 (%48) ve 2 (%4); prob hareketleri sırasında 34 (%68), 14 (%28) ve 2 (%4) ve biyopsi alımı sırasında ise 30 (%60), 14 (%28) ve 2 (%4) olarak gerçekleşti. Hasta memnuniyetinin dağılımı kötü 1 (%2), iyi 12 (%24) ve çok iyi 37(%74) şeklinde idi. Bromage skalası tüm hastalarda 0 olarak saptandı Üç hastada idrar retansiyonu gelişti. TRUS eşliğinde yapılan tru-cut prostat biyopsisi işleminden önce düşük konsantrasyonlu lokal anestezik ve opioid kombinasyonu ile yapılan kaudal anestezinin ağrıyı önlemede etkili ve güvenilir bir yöntemdir.
